【摘要】：目的探討經(jīng)后外側(cè)入路前后一體椎弓根釘棒系統(tǒng)內(nèi)固定治療胸腰椎骨折合并脊髓損傷的臨床療效。方法回顧性分析自2010-11—2014-11手術(shù)治療的70例胸腰椎爆裂骨折合并脊髓損傷,采用后外側(cè)入路減壓聯(lián)合椎弓根釘棒系統(tǒng)前后一體內(nèi)固定治療。結(jié)果 70例切口均一期愈合,均獲得平均18(12~36)個(gè)月隨訪。無內(nèi)固定松動(dòng)、椎弓根釘斷裂等并發(fā)癥發(fā)生。術(shù)后1年脊髓神經(jīng)功能Frankel分級(jí)均有不同程度改善。術(shù)后3 d傷椎Cobb角、傷椎前緣壓縮率、傷椎后緣壓縮率、椎管狹窄率較術(shù)前明顯改善,差異有統(tǒng)計(jì)學(xué)意義(P0.05);且末次隨訪時(shí)與術(shù)后3 d比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論經(jīng)后外側(cè)入路前后一體椎弓根釘棒系統(tǒng)內(nèi)固定技術(shù)具有創(chuàng)傷小,減壓徹底,良好恢復(fù)脊柱三柱穩(wěn)定性的優(yōu)點(diǎn),是治療胸腰椎爆裂骨折合并脊髓損傷的有效方法。
[Abstract]:Objective to investigate the clinical effect of anterior and posterior pedicle screw system internal fixation in the treatment of thoracolumbar fractures with spinal cord injury. Methods 70 cases of thoracolumbar burst fracture complicated with spinal cord injury treated from 2010-11 to 2014-11 were analyzed retrospectively. Posterior lateral approach decompression combined with anterior and posterior internal fixation of pedicle screw system was used to treat 70 cases of thoracolumbar burst fracture. Results all the 70 incisions healed at the first stage and were followed up for an average of 18 (12 ~ 36) months. No internal fixation loosening, pedicle screw fracture and other complications occurred. 1 year after operation, the Frankel grade of spinal cord nerve function was improved to some extent. The Cobb angle, anterior compression rate, posterior compression rate and stenosis rate of injured vertebrae were significantly improved 3 days after operation (P0.05). There was no significant difference between 3 days after operation and at the last follow-up (P 0.05). Conclusion the internal fixation technique of anterior and posterior pedicle screw system through posterolateral approach has the advantages of less trauma, complete decompression and good stability of three columns of spine. It is an effective method to treat thoracolumbar burst fracture with spinal cord injury.
