【摘要】：近年來,機(jī)器人手術(shù)系統(tǒng)被越來越多地應(yīng)用于肝臟外科手術(shù)中,其手術(shù)適應(yīng)證亦逐步擴(kuò)大。除傳統(tǒng)腹腔鏡肝切除術(shù)的一些創(chuàng)傷小和恢復(fù)快的優(yōu)勢外,機(jī)器人輔助腹腔鏡由于具備內(nèi)腕結(jié)構(gòu)和精細(xì)操作能力,更加適用于一些復(fù)雜的肝切除手術(shù)。目前有關(guān)機(jī)器人輔助腹腔鏡肝切除手術(shù)仍處于逐步完善階段,一些具體操作細(xì)節(jié)如體位、trocar布局、術(shù)中腫瘤定位、血流阻斷和肝實(shí)質(zhì)離斷等方面,均明顯有別于傳統(tǒng)腹腔鏡肝切除技術(shù)。盡管機(jī)器人輔助腹腔鏡肝切除術(shù)的近期效果與傳統(tǒng)腹腔鏡乃至開腹手術(shù)相當(dāng),但由于總體例數(shù)還有限,且觀察時限尚短,其長期治療效果還有待于進(jìn)一步評估。
[Abstract]:In recent years, robotic surgery system has been used more and more in liver surgery. In addition to the advantages of traditional laparoscopic hepatectomy with small trauma and quick recovery robot-assisted laparoscopic surgery is more suitable for complicated hepatectomy because of its internal carpal structure and fine operation ability. At present, the robot-assisted laparoscopic hepatectomy is still in the stage of improvement. Some specific operational details, such as posture, trocar layout, tumor location during operation, blood flow occlusion and hepatic parenchyma dissection, etc. All of them were different from the traditional laparoscopic hepatectomy technique. Although the short-term effect of robot-assisted laparoscopic hepatectomy is comparable to that of traditional laparoscopy and even open surgery, due to the limited number of cases and the short observation time, the long-term therapeutic effect needs to be further evaluated.
