【摘要】：目的探討血管腔內(nèi)技術(shù)在腹腔臟器急性動(dòng)脈出血中的應(yīng)用價(jià)值。方法對(duì)159例次急性消化道或腹腔急性出血的患者行腹腔動(dòng)脈造影檢查,對(duì)其中陽(yáng)性且無(wú)治療禁忌的40例次置入彈簧圈、注入明膠海綿顆粒、放置覆膜支架等方式行腔內(nèi)止血治療。通過(guò)術(shù)后造影、癥狀、生命體征、實(shí)驗(yàn)室檢查結(jié)果判斷止血是否成功。結(jié)果 47例次(47/159,29.56%)造影陽(yáng)性;外科術(shù)后出血77例次,造影陽(yáng)性率37.66%(29/77),非外科術(shù)后消化道出血82例次,造影陽(yáng)性率21.95%(18/82),兩者差異有統(tǒng)計(jì)學(xué)意義(P0.05)。造影陽(yáng)性患者共進(jìn)行40例次介入治療,其中置入彈簧圈33例次,注入明膠海綿顆粒4例次,置入覆膜支架3例次,腔內(nèi)止血成功率為100%(40/40)。結(jié)論血管腔內(nèi)技術(shù)是治療腹腔臟器急性動(dòng)脈出血的可靠手段。
[Abstract]:Objective to evaluate the value of endovascular technique in acute arterial hemorrhage of abdominal viscera. Methods A total of 159 patients with acute gastrointestinal or peritoneal hemorrhage were examined by celiac arteriography. 40 patients with positive and no contraindication were implanted with coils and injected with gelatin sponge granules. Endovascular hemostasis was performed by placement of covered stent. The success of hemostasis was judged by postoperative angiography, symptoms, vital signs and laboratory findings. Results 47 cases (47 / 159P 29.56%) were positive. The positive rate of angiography was 37.66% (29 / 77) and that of non-surgical gastrointestinal bleeding was 21.95% (18 / 82). The difference was statistically significant (P0.05). A total of 40 interventional treatments were performed in 40 patients with positive angiography, of which 33 were placed with coils, 4 were injected with gelatin sponge granules, and 3 were implanted with coated stents. The success rate of endovascular hemostasis was 100% (40 / 40). Conclusion Endovascular technique is a reliable method for the treatment of acute arterial hemorrhage in abdominal viscera.
