【摘要】：目的:探討膜聯(lián)蛋白A2(annexin A2)與上皮-間質(zhì)轉(zhuǎn)化(EMT)標(biāo)志物上皮細胞鈣黏蛋白(E-cadherin)及波形蛋白(vimentin)在肝內(nèi)膽管結(jié)石相關(guān)肝內(nèi)膽管癌中的表達及意義。方法:用免疫組化檢測46例肝內(nèi)膽管結(jié)石相關(guān)肝內(nèi)膽管癌組織(腫瘤組)、50例單純肝內(nèi)膽管結(jié)石慢性炎癥膽管組織(炎癥組)及35例來自血管瘤旁和肝外傷的正常膽管組織(正常組)中annexin A2、E-cadherin、vimentin的表達,比較各組3種蛋白表達的差異,并分析3種蛋白的表達與腫瘤組患者臨床病理特征及預(yù)后的關(guān)系。結(jié)果:annexin A2與vimentin的陽性表達率在腫瘤組、炎癥組、正常組均呈依次明顯降低(69.6%vs.36.0%vs.11.4%;54.3%vs.28.0%vs.8.6%,均P0.05),而E-cadherin的陽性表達率則相反(21.7%vs.48.0%vs.71.4%,均P0.05);在膽管癌組織中,annexin A2與vimentin的表達呈正相關(guān)(r=0.627,P0.05),E-cadherin的表達與annexin A2、vimentin的表達均呈負相關(guān)(r=-0.682;r=-0.575,均P0.05)。annexin A2、vimentin的表達與腫瘤的分化程度、淋巴結(jié)轉(zhuǎn)移、TNM分期有關(guān),E-cadherin的表達與腫瘤的分化程度、淋巴結(jié)轉(zhuǎn)移有關(guān)(均P0.05)。肝內(nèi)膽管結(jié)石相關(guān)肝內(nèi)膽管癌患者中,annexin A2和vimentin各自陽性者的生存率低于其各自陰性者,E-cadherin陰性者生存率低于其陽性者(均P0.05)。結(jié)論:annexin A2/EMT通路在肝內(nèi)膽管結(jié)石相關(guān)肝內(nèi)膽管癌的發(fā)生和發(fā)展中可能至關(guān)重要,annexin A2、E-cadherin及vimentin的表達與腫瘤的惡性程度和患者預(yù)后密切相關(guān)。
[Abstract]:Objective: to investigate the expression and significance of annexin A2 and epithelial cell cadherin (E-cadherin) and vimentin (vimentin) in intrahepatic cholelithiasis associated with intrahepatic cholangiocarcinoma. Methods: 46 cases of intrahepatic bile duct calculi associated with intrahepatic cholangiocarcinoma (tumor group) were detected by immunohistochemistry. The expression of annexin A _ 2 E-cadherin vimentin was detected in 50 cases of chronic inflammatory bile duct tissue (inflammation group) and 35 cases of normal bile duct tissue (normal group) from hemangioma and hepatic trauma. The relationship between the expression of three proteins and the clinicopathological features and prognosis of tumor patients was analyzed. Results: the positive expression rates of annexin A2 and vimentin in tumor group, inflammatory group and normal group were significantly decreased in turn (69.6 vs.36.0vs.11.4). The positive expression rate of E-cadherin was opposite (21.7vs.48.0 vs 71.4, P0.05). There was a positive correlation between the expression of annexin A2 and vimentin (r 0.627 P 0.05), and a negative correlation between the expression of E-cadherin and the expression of annexin A 2 vimentin (r = 0.682). The expression of). Annexin A2vimentin was related to the degree of differentiation, lymph node metastasis and TNM stage, and the expression of E-cadherin was related to the degree of differentiation and metastasis of lymph node (P0.05). In patients with intrahepatic cholelithiasis associated intrahepatic cholangiocarcinoma, the survival rate of annexin A2 and vimentin positive patients was lower than that of their respective negative patients, and the survival rate of E-cadherin negative patients was lower than that of E-cadherin negative patients (P0.05). Conclusion: the annexin A2/EMT pathway may play an important role in the occurrence and development of intrahepatic cholangiocarcinoma associated with intrahepatic cholelithiasis. The expression of annexin A2E-cadherin and vimentin is closely related to the malignancy of the tumor and the prognosis of the patients.
