【摘要】：目的分析切開掛線聯(lián)合甲硝唑、左氧氟沙星對肛周膿腫感染患者血漿內(nèi)毒素水平及病原菌的影響。方法選擇2015年9月-2016年9月期間醫(yī)院收治的肛周膿腫感染患者70例,按照隨機(jī)、對照、雙盲試驗(yàn)原則分為試驗(yàn)組和對照組,每組各35例,對照組患者采用甲硝唑聯(lián)合左氧氟沙星治療,在對照組治療基礎(chǔ)上,試驗(yàn)組患者聯(lián)合切開掛線治療,觀察分析病原菌、血漿內(nèi)毒素、治療有效率、復(fù)發(fā)率等指標(biāo)評估臨床療效。結(jié)果治療后,試驗(yàn)組患者真菌、金黃色葡萄球菌、大腸埃希菌病原菌菌落數(shù)均低于對照組(P0.05);試驗(yàn)組患者血漿內(nèi)毒素水平低于對照組(P0.05);試驗(yàn)組患者治療總有效率91.43%,高于對照組68.57%(P0.05);試驗(yàn)組復(fù)發(fā)率為2.86%,低于對照組11.43%(P0.05)。結(jié)論對肛周膿腫感染患者,切開掛線聯(lián)合甲硝唑、左氧氟沙星治療,有利于改善肛周病原菌數(shù)量,防止血漿內(nèi)毒素水平升高,提高臨床治療效果,降低復(fù)發(fā)率,值得臨床積極推廣和應(yīng)用。
[Abstract]:Objective to analyze the effect of incision and thread combined with metronidazole and levofloxacin on plasma endotoxin level and pathogenic bacteria in patients with perianal abscess infection. Methods from September 2015 to September 2016, 70 patients with perianal abscess infection were selected. According to the principles of randomized, controlled and double-blind trials, they were divided into two groups, 35 cases in each group. The patients in the control group were treated with metronidazole combined with levofloxacin. On the basis of the treatment in the control group, the patients in the experimental group were treated with incision and thread, and the pathogenic bacteria, plasma endotoxin, effective rate and recurrence rate were observed and analyzed. Results after treatment, the number of fungi, Staphylococcus aureus and Escherichia coli in the experimental group was lower than that in the control group (P0.05), and the plasma endotoxin level in the experimental group was lower than that in the control group (P0.05). The total effective rate of treatment in the trial group was 91.43, which was higher than that in the control group (68.57%) (P0.05), and the recurrence rate in the trial group was 2.86%, lower than that in the control group (11.43%) (P0.05). Conclusion for the patients with perianal abscess infection, the treatment of incision combined with metronidazole and levofloxacin is beneficial to improve the number of pathogenic bacteria around the anus, prevent the increase of plasma endotoxin level, improve the clinical treatment effect and reduce the recurrence rate. It is worth popularizing and applying in clinic.
