【摘要】：背景:氨甲環(huán)酸局部應(yīng)用于全髖關(guān)節(jié)置換可顯著減少圍手術(shù)期失血量,然而一直以來(lái)的研究重點(diǎn)在于關(guān)節(jié)置換后總引流量方面,對(duì)于置換后不同時(shí)間段內(nèi)的引流量觀察甚少。目的:觀察局部應(yīng)用氨甲環(huán)酸對(duì)全髖關(guān)節(jié)置換后不同時(shí)間的引流量的影響。方法:回顧性分析初次行單側(cè)全髖關(guān)節(jié)置換的病例資料共79例,分為2組。試驗(yàn)組38例關(guān)節(jié)置換后經(jīng)引流管逆行灌注氨甲環(huán)酸1.5 g(50 m L),對(duì)照組41例以同樣方法灌注50 m L生理鹽水。統(tǒng)計(jì)2組關(guān)節(jié)置換后第1,2,3天引流量、總引流量、置換后第3天血紅蛋白。結(jié)果與結(jié)論:2組基線資料差異無(wú)統(tǒng)計(jì)學(xué)意義。(1)試驗(yàn)組置換后第1天引流量顯著小于對(duì)照組(P0.05),第2,3天2組引流量差異無(wú)顯著性意義(P0.05);試驗(yàn)組總引流量顯著小于對(duì)照組(P0.05);(2)試驗(yàn)組置換后第3天血紅蛋白顯著高于對(duì)照組(P0.05);(3)結(jié)果表明,局部應(yīng)用氨甲環(huán)酸可減少初次單側(cè)全髖關(guān)節(jié)置換后引流量,尤以置換后第1天明顯。
[Abstract]:Background: total hip replacement can significantly reduce blood loss in total hip replacement by local application of aminocyclylic acid. However, the focus of the study has been on the total drainage after arthroplasty, and little observation has been made on the drainage in different periods after replacement. Aim: to observe the effect of local application of carbamicylic acid on the drainage flow after total hip replacement (THR). Methods: 79 cases of primary total hip replacement were analyzed retrospectively and divided into 2 groups. After joint replacement, 38 patients in the test group were retrograde perfused with methocycline 1.5 g through drainage tube. 41 cases in 50 m L), control group were perfused with 50 mL normal saline in the same way. The drainage flow and the hemoglobin were calculated on the 1st day and 2nd day after arthroplasty in the two groups. Results and conclusion: there was no significant difference in baseline data between the two groups. (1) the drainage volume in the experimental group was significantly lower than that in the control group on the first day after replacement (P0.05), but there was no significant difference between the two groups on the 2nd day (P0.05). The total drainage volume in the trial group was significantly lower than that in the control group (P0.05); (2). The hemoglobin in the experimental group was significantly higher than that in the control group on the 3rd day after replacement (P0.05). (3) the results showed that local application of carbamoic acid could reduce the drainage volume after the first unilateral total hip replacement, especially on the first day after replacement.
