【摘要】：目的探討肝內(nèi)膽管結(jié)石患者采用肝葉切除術(shù)治療后結(jié)石復(fù)發(fā)的危險因素。方法選取2008年1月—2015年1月本院收治的174例行肝葉切除術(shù)治療的患者,對患者的病歷資料進(jìn)行回顧性分析。按照膽總管結(jié)石復(fù)發(fā)標(biāo)準(zhǔn)分為復(fù)發(fā)組和非復(fù)發(fā)組,通過單因素和多因素分析確定肝內(nèi)膽管結(jié)石復(fù)發(fā)的危險因素。結(jié)果 174例患者中有167例成功獲得確切隨訪,有24例患者肝內(nèi)膽管結(jié)石復(fù)發(fā),復(fù)發(fā)率為14.4%。膽總管直徑15 mm[OR=5.162,95%CI(2.582,17.256)]、肝內(nèi)結(jié)石≥10 mm[OR=2.687,95%CI(1.421,4.452)]、多處結(jié)石[OR=3.214,95%CI(1.542,5.286)]及合并膽道炎癥[OR=1.986,95%CI(1.214,4.159)]是肝葉切除術(shù)后肝內(nèi)膽管結(jié)石復(fù)發(fā)的影響因素(P0.05)。結(jié)論膽總管直徑15 mm、肝內(nèi)結(jié)石10 mm、多處結(jié)石及合并膽道炎癥是肝葉切除術(shù)治療后肝內(nèi)膽管結(jié)石復(fù)發(fā)的危險因素。
[Abstract]:Objective to investigate the risk factors of calculi recurrence after hepatectomy in patients with intrahepatic cholelithiasis. Methods 174 patients treated with lobectomy from January 2008 to January 2015 were retrospectively analyzed. According to the criteria of choledocholithiasis recurrence was divided into recurrent group and non-recurrent group. The risk factors of intrahepatic cholelithiasis recurrence were determined by univariate and multivariate analysis. Results 167 of 174 cases were followed up successfully, 24 cases of intrahepatic cholelithiasis recurred, the recurrence rate was 14.4%.
