【摘要】：目的:比較I-gel喉罩和標準型喉罩用于保留自主呼吸下單孔胸腔鏡肺大皰切除術中氣道管理的效果。方法:選擇擇期行單孔胸腔鏡肺大皰切除術患者40例,隨機分為I-gel喉罩組(I組)和標準型喉罩組(B組),麻醉誘導后分別置入I-gel喉罩和標準型喉罩;記錄兩組患者建立通氣的時間、首次置入成功率、口咽腔漏氣壓、健側臥位纖維支氣管鏡評級、術中調整喉罩位置例數(shù)及術后24 h內相關并發(fā)癥。結果:與I組比較,B組建立通氣時間較長(P0.05),首次置入成功例數(shù)較少(P0.05),口咽腔漏氣壓較低(P0.05);I組纖維支氣管鏡評級為Ⅰ級者少于B組(P0.05);健側臥位下I組調整喉罩位置例數(shù)少于B組(P0.05);術后24 h內咽痛患者I組少于B組(P0.05)。結論:保留自主呼吸下單孔胸腔鏡肺大皰切除術中,I-gel喉罩與標準型喉罩相比氣道管理效果更佳,且并發(fā)癥較少,更加便于氣道管理。
[Abstract]:Objective: to compare the effect of I-gel laryngeal mask and standard laryngeal mask on airway management in patients undergoing thoracoscopic pneumonectomy. Methods: forty patients were randomly divided into I-gel laryngeal mask group (group I) and standard laryngeal mask group (group B). After anesthesia induction, I-gel larynx mask and standard larynx mask were implanted respectively. The time of establishing ventilation, the successful rate of first placement, the air leakage pressure of oropharyngeal cavity, the grading of fiberoptic bronchoscopy in the healthy lateral position, the cases of adjusting the position of larynx mask during operation and the related complications within 24 hours after operation were recorded. Results: compared with group I, the time of establishing ventilation in group B was longer (P0.05), the number of successful first placement was less (P0.05), the air pressure of oropharyngeal cavity leakage was lower (P0.05) than that of group B (P0.05), and the position of larynx mask was adjusted in group I under healthy lateral position (P0.05). The number of cases in group B was less than that in group B (P0.05), and that in group I was less than that in group B within 24 hours after operation (P0.05). Conclusion: compared with standard larynx mask, I-gel larynx mask is more effective in airway management than standard laryngeal mask, and has fewer complications and is more convenient for airway management.
