【摘要】：目的:觀察桃紅四物湯對肩胛骨骨折術(shù)后,肩背部疼痛、腫脹,肩關(guān)節(jié)功能活動受限等功能障礙的改善情況,進一步驗證桃紅四物湯在骨折早期的應(yīng)用和推廣。方法:共收集的病例21例,隨機分成治療組和對照組。治療組和對照組手術(shù)均是在全麻下,采用改良的Judet手術(shù)入路法手術(shù)。兩組患者均于術(shù)后拍片檢查內(nèi)固定物是否牢固,選用方巾將患肢前臂行胸前懸吊。對照組術(shù)后常規(guī)靜脈點滴活血化瘀及消腫的中成藥,治療組在術(shù)后第2天開始加用口服桃紅四物湯,組方:生地15g、當歸15g、赤芍10g、川芎8g、桃仁9g、紅花6g。將各味中藥加水250 ml沒過草藥,浸泡2小時,用武火煎煮15分鐘,取藥汁150 ml,繼續(xù)再加水200 ml,武火煎煮20分鐘,取藥汁150ml,把兩次取出的藥汁相兌。一日一劑,早晚兩次空腹口服,連服7天為1個療程,持續(xù)3個療程。結(jié)果::1.、治療組較對照組患者肩背部疼痛在術(shù)后第3、5天疼痛值低,有差異(P0.05);2.、治療組較對照組患者在術(shù)后第3、5天腫脹消退程度快,有差異(P0.05)。3.、術(shù)后第1、2、3月患肢肩關(guān)節(jié)功能評分有差異(P0.05),且治療組分值較對照組大,說明治療組在術(shù)后肩關(guān)節(jié)功能恢復(fù)情況較對照組好。4.、治療組肩關(guān)節(jié)優(yōu)良率明顯高于對照組。結(jié)論:本研究表明桃紅四物湯結(jié)合改良Judet手術(shù)入路法對肩胛骨骨折的康復(fù)優(yōu)于單純改良Judet手術(shù)入路法治療肩胛骨骨折。桃紅四物湯在肩胛骨骨折術(shù)后康復(fù)過程中,起到止痛、消腫,促進肩關(guān)節(jié)功能恢復(fù)的作用,對骨折早期的功能恢復(fù)具有顯著的療效。
[Abstract]:Objective: to observe the improvement of Taohong Siwu decoction on shoulder and back pain, swelling and limited function of shoulder joint after scapular fracture, and to verify the application and popularization of Taohong Siwu decoction in the early stage of fracture. Methods: 21 cases were randomly divided into treatment group and control group. The treatment group and control group were operated under general anesthesia with modified Judet approach. The patients in both groups were examined by radiographs after operation to see if the internal fixation was firm and the forearm of the affected limb was suspended by using a square towel. In the control group, the traditional Chinese patent medicine of promoting blood circulation and removing blood stasis and reducing swelling was given by intravenous drip after operation. In the treatment group, oral Taohong Siwu decoction was added on the second day after operation. The prescription: Shengdi 15g, Angelica sinensis 15g, Radix Paeoniae rubra 10g, Chuanxiong 8g, peach kernel 9g, safflower 6g. The herbs were soaked in water for 2 hours and boiled for 15 minutes. The juice was further boiled for 20 minutes by adding water for 200 ml,. The medicine juice was added for 150ml and the juice was mixed twice. One dose a day, two fasting oral morning and evening, 7 days as a course of treatment, continued for 3 courses. Results: 1. The pain value of shoulder and back pain in the treatment group was lower than that in the control group on the 5th day after operation (P0.05); 2. The swelling degree of the treatment group was faster than that of the control group on the 3rd day after operation. There was a difference (P0.05). 3. The scores of shoulder joint function in the first and third months after operation were different (P0.05), and the score of the treatment group was larger than that of the control group, which indicated that the recovery of the shoulder joint function in the treatment group was better than that in the control group. 4. The excellent and good rate of the shoulder joint in the treatment group was significantly higher than that in the control group. Conclusion: this study shows that Taohong Siwu decoction combined with modified Judet operative approach is superior to modified Judet surgical approach in the rehabilitation of scapular fractures. Taohong Siwu decoction can relieve pain, reduce swelling and promote the recovery of shoulder joint function in the recovery process of scapular fracture.
