【摘要】：目的比較經(jīng)腋窩入路與經(jīng)胸乳入路腔鏡甲狀腺切除術(shù)的臨床應(yīng)用價(jià)值。方法我院2013年1月~2016年1月手術(shù)治療201例單側(cè)甲狀腺良性疾病,根據(jù)患者意愿,其中96例采用經(jīng)腋窩入路,105例采用經(jīng)胸乳入路,術(shù)中均不常規(guī)顯露喉返神經(jīng)。比較2組手術(shù)時間、出血量、引流量、住院時間、術(shù)后并發(fā)癥。結(jié)果手術(shù)均獲成功。與經(jīng)胸乳組相比,經(jīng)腋窩組手術(shù)時間短[(56.9±13.6)min vs.(71.4±11.3)min,t=-8.258,P=0.000],術(shù)中出血少[(14.0±8.5)ml vs.(21.0±12.5)ml,t=-4.553,P=0.000],術(shù)后引流少[(33.5±14.4)ml vs.(51.1±22.0)ml,t=-6.630,P=0.000]。2組住院時間差異無顯著性[(5.3±0.9)d vs.(5.4±1.1)d,t=-0.475,P=0.635]。2組均無喉返神經(jīng)損傷及甲狀旁腺損傷等并發(fā)癥。結(jié)論經(jīng)腋窩及胸乳入路的腔鏡甲狀腺切除術(shù)是一種安全可行的方法,但經(jīng)腋窩入路創(chuàng)傷程度及手術(shù)時間優(yōu)于經(jīng)胸乳入路。
[Abstract]:Objective to compare the clinical value of transaxillary approach and transthoracic approach for endoscopic thyroidectomy. Methods 201 cases of unilateral benign thyroid diseases were operated from January 2013 to January 2016. According to the wishes of the patients, 96 cases were treated by transaxillary approach and 105 cases by transthoracic mammary approach. The recurrent laryngeal nerve was not routinely exposed during the operation.
