【摘要】：背景:老年股骨粗隆間骨折的發(fā)生率正呈上升趨勢,對其行內(nèi)固定術治療的術后愈合效果不佳,值得臨床醫(yī)師探索一種新的治療方式。方法:自2004年3月到2014年11月,蘇州大學附屬第一醫(yī)院骨關節(jié)科為20例老年股骨粗隆間骨折患者施行了人工雙極股骨頭置換術。假體材料為德國LINK公司設計,生物相容性較佳。結果與討論:隨訪時間6個月—10年2個月,術后療效均較滿意,假體置換術后早期便可離床活動,髖關節(jié)活動度良好,患者能盡快恢復到骨折前的生活狀態(tài),生活質量得以保證。對術后隨訪6個月以上的20例患者行HARRIS評分顯示,好、優(yōu)18例,良2例,可0例,差0例。結果顯示:人工雙極股骨頭置換在應用于老年股骨粗隆間骨折的治療時可明顯縮短患者的術后臥床時間,有利于早期離床進行功能鍛煉,恢復效果佳,是一種相對安全且有效的可選治療方法。結論:人工雙極股骨頭置換術在應用于老年股骨粗隆間骨折的治療時療效明確,可有效地避免內(nèi)固定術帶來的內(nèi)固定失效及復位失敗等并發(fā)癥,且固定穩(wěn)定性較高。同時,假體置換明顯地提前了患者的離床活動時間,縮短了患者的康復周期,可使骨折患者盡早恢復正常的生活狀態(tài),患者術后滿意度較高。
[Abstract]:Background: the incidence of intertrochanteric fractures in the elderly is on the rise. Methods: from March 2004 to November 2014, the Department of Orthopedics and Joint of the first affiliated Hospital of Suzhou University performed artificial bipolar femoral head replacement in 20 elderly patients with intertrochanteric fracture of femur. The prosthesis material is designed by LINK company in Germany, and its biocompatibility is better. Results and discussion: the follow-up time was 6 months to 10 years and 2 months. The curative effect was satisfactory. The patients could move out of bed early after prosthesis replacement, the hip joint motion was good, the patient could recover to the life state before fracture as soon as possible. The quality of life is guaranteed. The HARRIS score showed good, excellent in 18 cases, good in 2 cases, fair in 0 cases and poor in 0 cases. The results showed that the artificial bipolar femoral head replacement in the treatment of intertrochanteric fracture of the femur could significantly shorten the bed-rest time of the patients after operation, which was beneficial to the early functional exercise out of bed, and the recovery effect was good. Is a relatively safe and effective alternative treatment. Conclusion: the artificial bipolar femoral head replacement is effective in the treatment of intertrochanteric fracture of the femur in the elderly. It can effectively avoid the complications such as the failure of internal fixation and the failure of reduction, and the stability of fixation is relatively high. At the same time, prosthesis replacement can obviously advance the time of the patients' off-bed activities, shorten the rehabilitation period of the patients, and make the fracture patients return to normal life as soon as possible, and the patients' satisfaction after operation is higher.
