【摘要】：目的觀察上肢神經(jīng)卡壓患者的神經(jīng)超聲圖像特征,明確卡壓神經(jīng)除受壓處損害外是否有其他部位的合并性損害,研究卡壓神經(jīng)合并性損害與性別及卡壓點(diǎn)橫截面積(CSA)的關(guān)系。方法回顧性總結(jié)2013年10月-2015年10月在貴州醫(yī)科大學(xué)附屬醫(yī)院神經(jīng)科確診為腕管綜合征(CTS)、肘管綜合征(Cub TS)患者(49例)的高頻超聲圖像資料。將卡壓神經(jīng)分為CTS伴CSA增粗組、CTS伴CSA正常組、Cub TS伴CSA增粗組和Cub TS伴CSA正常組,統(tǒng)計(jì)各組卡壓神經(jīng)合并性損害的情況,分析各組出現(xiàn)合并性損害與性別及卡壓點(diǎn)CSA的關(guān)系。結(jié)果 79條卡壓神經(jīng)中有48條合并該神經(jīng)其他部位損害。CTS伴CSA增粗組中有合并性損害與無(wú)合并性損害的神經(jīng)卡壓點(diǎn)CSA比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);CTS是否同時(shí)伴有CSA增粗,其性別間比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論卡壓性神經(jīng)損害除卡壓點(diǎn)外,可能涉及神經(jīng)其他部位,CTS出現(xiàn)合并性損害可能與卡壓點(diǎn)神經(jīng)增粗程度有關(guān)。神經(jīng)有卡壓不一定有卡壓點(diǎn)神經(jīng)腫脹,性別可能是影響神經(jīng)卡壓后是否出現(xiàn)卡壓點(diǎn)神經(jīng)腫脹的一個(gè)因素。高頻超聲對(duì)神經(jīng)卡壓的有重要診斷意義,對(duì)其早期診斷有潛在價(jià)值。
[Abstract]:Objective to observe the characteristics of nerve ultrasound images in patients with nerve compression of upper limb, to determine whether there are combined lesions in other parts of the compressed nerve, and to study the relationship between the combined damage of compression nerve and sex and cross sectional area of compression spot (CSA). Methods from October 2013 to October 2015, 49 patients (49 cases) with carpal tunnel syndrome (CTS),) and cubital tunnel syndrome (Cub TS) diagnosed in the Department of Neurology, affiliated Hospital of Guizhou Medical University, were retrospectively analyzed. The compressed nerves were divided into CTS with CSA thickening group, CTS with CSA normal group, Cub TS with CSA thickening group and Cub TS with CSA normal group. Results 48 of 79 compressed nerves were associated with other lesions of the nerve. There was a significant difference in CSA between the CTS with CSA thickening group and the nerve compression point CSA without combined lesion (P0.05); CTS or not accompanied by CSA thickening at the same time). The difference was statistically significant (P0.05). Conclusion the nerve damage caused by compression may involve other parts of the nerve except the compression point. The occurrence of combined damage in CTS may be related to the degree of nerve thickening at the point of compression. Nerve entrapment does not necessarily have compression point nerve swelling, sex may be one of the factors that affect the nerve swelling after nerve compression. High frequency ultrasound has important diagnostic significance for nerve compression, and has potential value for early diagnosis.
