【摘要】：[目的]探討血清B型尿鈉肽(BNP)、心肌肌鈣蛋白T(c Tn T)檢測(cè)在老年髖部骨折圍手術(shù)期心臟評(píng)估的意義。[方法]2010年1月~2015年1月于本院進(jìn)行治療的142例髖部骨折患者分為低齡組和高齡組,將兩組患者術(shù)前24 h及術(shù)后1、3 d血清BNP、c Tn T水平進(jìn)行檢測(cè)及比較。[結(jié)果]兩組BNP檢測(cè)結(jié)果顯示:低年齡組及高年齡組術(shù)后1、3 d均較術(shù)前1 d升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05),高年齡組在相同時(shí)間點(diǎn)明顯高于低年齡組,尤其是術(shù)后變化更加明顯,相同時(shí)間點(diǎn)兩組間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組c Tn T測(cè)量結(jié)果顯示:低年齡組及高年齡組術(shù)后1、3 d均較術(shù)前1 d升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后高年齡組在相同時(shí)間點(diǎn)明顯高于低年齡組,術(shù)后3 d兩組間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)前1 d、術(shù)后1 d高年齡組有升高趨勢(shì),但相同時(shí)間點(diǎn)兩組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)�；颊吣挲g與血清BNP、c Tn T檢測(cè)值的Pearson相關(guān)分析顯示具有一定相關(guān)性。[結(jié)論]血清BNP、c Tn T是敏感的心肌梗死及心力衰竭標(biāo)志物,二者在老年髖部骨折圍手術(shù)期心臟評(píng)估中動(dòng)態(tài)變化價(jià)值較高,骨折患者隨其年齡及手術(shù)升高變化,具有一定規(guī)律性。
[Abstract]:[objective] to investigate the clinical significance of cardiac troponin T (c Tn T) detection in elderly patients with hip fracture by serum type B urinary natriuretic peptide (BNP),). [methods] 142 patients with hip fracture treated in our hospital from January 2010 to January 2015 were divided into two groups: the low age group and the old age group. The serum BNP,c Tn T levels were measured and compared between the two groups 24 hours before operation and 1: 3 days after operation. [results] the results of BNP detection in the two groups showed that the level of BNP in the low age group and the high age group was significantly higher than that in the preoperation 1 day after operation (P0.05). The changes in the high age group were significantly higher than those in the low age group at the same time point, especially after the operation. There was significant difference between the two groups at the same time (P0.05). The results of c Tn T measurement in the two groups showed that 1 day after operation, 3 days after operation in the low age group and the high age group were significantly higher than that in the low age group (P 0.05), and the difference was statistically significant (P0.05). At the same time after operation, the high age group was significantly higher than the low age group. There was a significant difference between the two groups 3 days after operation (P0.05), 1 day before surgery, 1 day after the operation of the high age group has an upward trend, but at the same time point between the two groups there was no significant difference (P0.05). The Pearson correlation analysis of age and serum BNP,c Tn T showed a certain correlation. [conclusion] Serum BNP,c Tn T is a sensitive marker of myocardial infarction and heart failure. The dynamic changes of serum BNP,c Tn T in elderly patients with hip fracture in perioperative period are higher.
