【摘要】：目的:從1822年(最早有文獻(xiàn)記載的“毛發(fā)移植術(shù)”)至今,目前世界上毛發(fā)移植術(shù)的兩大方法是毛囊單位移植術(shù)(FUT)和毛囊單位抽取術(shù)(FUE),而FUE在毛發(fā)移植中逐漸占有重要的地位,我科近年在兩大治療毛發(fā)缺失的方法中,探索分析手術(shù)中的主要步驟及其優(yōu)缺點(diǎn),尋找治療毛發(fā)缺失的理想方法。方法:選取不同原因引起體表毛發(fā)缺失患者11名(其中運(yùn)用FUT治療5名,FUE治療4名,FUT+FUE聯(lián)合治療2名),監(jiān)控毛發(fā)移植的每個(gè)環(huán)節(jié)(毛發(fā)供區(qū)的范圍、手術(shù)中毛囊的提取及保存等),并通過(guò)比較其手術(shù)時(shí)間及人員配備、術(shù)后效果及毛發(fā)存活率、患者的術(shù)后反饋等項(xiàng)目來(lái)評(píng)估治療方法。結(jié)果:運(yùn)用FUT治療的毛發(fā)缺失患者,術(shù)后效果可,少數(shù)患者要求再次手術(shù)加密,手術(shù)過(guò)程中人員配置約4-6人,而且耗時(shí)、耗神,病人手術(shù)后反饋有手術(shù)切口,術(shù)后需要換藥拆線及遺留線性疤痕等;運(yùn)用FUE治療毛發(fā)缺失的患者,效果尚可,大部分在一期手術(shù)后要求再次手術(shù)加密,手術(shù)中器械會(huì)損傷部分毛囊單位,術(shù)中人員配置至少約2-3人,術(shù)后患者反饋雖無(wú)需換藥及拆線,但手術(shù)時(shí)間較長(zhǎng)。結(jié)論:目前世界上兩大毛發(fā)移植術(shù)在毛發(fā)缺失的治療中,各有優(yōu)缺點(diǎn),術(shù)后均有部分患者要求再次行手術(shù)加密,通過(guò)探討及分析其各優(yōu)缺點(diǎn),為患者選擇最優(yōu)的治療方式(在大面積毛發(fā)缺失患者中兩種技術(shù)的綜合治療可取得較好的效果),盡量通過(guò)一次性手術(shù)達(dá)到患者要求及臨床上的理想效果。但無(wú)論選擇哪種治療方案,在毛發(fā)移植的過(guò)程中均需要醫(yī)生具有足夠的耐心、責(zé)任感及專業(yè)的技術(shù)。
[Abstract]:Objective: from 1822 (the earliest documented "hair transplantation") until now, the two major methods of hair transplantation in the world are hair follicle unit transplantation (FUT) and hair follicle unit extraction (FUE),). FUE gradually occupies an important position in hair transplantation. In recent years, among the two major methods of treating hair loss, our department has explored and analyzed the main steps, advantages and disadvantages of the operation, and looked for an ideal method for the treatment of hair loss. Methods: 11 patients with body surface hair loss caused by different causes (5 patients treated with FUT and 2 patients treated with FUE combined with, FUT FUE) were selected to monitor each stage of hair transplantation (the area of hair donor area). The extraction and preservation of hair follicles during operation) and the evaluation of treatment methods by comparing the operation time and staffing, postoperative effect and hair survival rate, postoperative feedback of patients, etc. Results: the postoperative effect of the patients with hair loss treated with FUT was good. A few patients required re-operation encryption. During the operation, the staffing was about 4-6 people, and it was time consuming and energy consuming. The patient feedback had an incision after operation. After the operation, we need to change the dressing and remove the thread and leave a linear scar. The effect of using FUE to treat the patients with hair loss is fairly good. Most of the patients require re-operation after the first stage operation, and the instruments will damage some hair follicle units during the operation. The operative staffing was at least 2-3. The postoperative feedback was not needed, but the operation time was longer. Conclusion: at present, the two major hair transplantation in the world have their own advantages and disadvantages in the treatment of hair loss. To select the best treatment for patients (in large areas of hair loss patients can be achieved by two techniques of comprehensive treatment), as far as possible through one-off surgery to achieve patient requirements and clinical ideal results. However, whatever treatment is chosen, the patient, responsible and professional skills of the doctor are required in the hair transplant process.
