【摘要】：目的觀察后路短節(jié)段椎弓根螺釘固定聯(lián)合傷椎置釘治療胸腰椎爆裂性骨折患者的臨床療效。方法將60例胸腰椎爆裂性骨折患者隨機(jī)分為觀察組和對照組30例,均行后路短節(jié)段椎弓根螺釘固定治療,觀察組另行傷椎置釘。記錄手術(shù)時(shí)間、出血量及術(shù)后住院時(shí)間、術(shù)后并發(fā)癥發(fā)生情況,術(shù)后第6個(gè)月行Oswestry功能障礙指數(shù)評價(jià),采用脊柱X線片、CT測定患者術(shù)前、術(shù)后即刻及術(shù)后第6個(gè)月傷椎前緣高度、Cobb角。結(jié)果兩組手術(shù)時(shí)間、出血量、術(shù)后住院時(shí)間、術(shù)后并發(fā)癥發(fā)生率、術(shù)后傷椎高度矯正率、術(shù)后Cobb角矯正率、術(shù)后6個(gè)月Oswestry功能障礙指數(shù)比較差異無統(tǒng)計(jì)學(xué)意義。觀察組術(shù)后6個(gè)月傷椎高度矯正丟失率及Cobb角矯正丟失率均低于對照組(P均0.05)。結(jié)論后路短節(jié)段椎弓螺釘固定聯(lián)合傷椎置釘有利于恢復(fù)及保持胸腰椎爆裂性骨折患者的椎體高度,減少傷椎高度矯正丟失。
[Abstract]:Objective to observe the clinical effect of posterior short segmental pedicle screw fixation combined with injured spinal screw in the treatment of thoracolumbar burst fracture. Methods 60 patients with thoracolumbar burst fracture were randomly divided into observation group and control group. The time of operation, the amount of blood loss, the time of hospitalization, the incidence of postoperative complications, the evaluation of Oswestry dysfunction index at the 6th month after operation, and the X-ray of spine were measured by CT before operation. The anterior edge height and Cobb angle of the injured vertebrae were immediately after operation and 6 months after operation. Results there was no significant difference between the two groups in operation time, bleeding volume, postoperative hospitalization time, postoperative complication rate, postoperative correction rate of vertebral height, Cobb angle correction rate and Oswestry dysfunction index at 6 months after operation. The loss rate of vertebral height correction and Cobb angle correction in the observation group were lower than those in the control group 6 months after operation (P < 0. 05). Conclusion posterior short segmental pedicle screw fixation combined with traumatic screw insertion is beneficial to the recovery and maintenance of vertebral body height in patients with thoracolumbar burst fracture and reduces the loss of correction of injured vertebral height.
