丁丙諾啡透皮貼劑對老年股骨轉(zhuǎn)子間骨折術(shù)后的鎮(zhèn)痛效果與安全性

【摘要】：目的觀察老年股骨轉(zhuǎn)子間骨折術(shù)后使用丁丙諾啡透皮貼劑止痛臨床效果。方法將67例老年股骨轉(zhuǎn)子間骨折術(shù)后患者分為試驗組34例和對照組33例。試驗組術(shù)后予以丁丙諾啡透皮貼劑,對照組術(shù)后予以同等包裝不含丁丙諾啡藥用成分的空白貼劑。術(shù)后第1,2,3,4,5,6,7天,記錄2組患者收縮壓、舒張壓、心率、視覺模擬評分(VAS),同時統(tǒng)計藥物不良反應(yīng)。結(jié)果治療后第2,3,4,5天,試驗組的VAS分別為(6.69±1.83),(4.22±1.56),(3.73±2.20),(3.13±1.58);對照組的VAS分別為(8.15±1.36),(7.75±2.26),(7.14±1.67),(6.82±2.24),組間比較差異有統(tǒng)計學(xué)意義(P0.05)。試驗組的藥物不良反應(yīng)主要有惡心嘔吐、皮疹,對照組的藥物不良反應(yīng)主要有惡心嘔吐,2組藥物不良反應(yīng)發(fā)生率分別為23.53%和6.06%,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論研究表明丁丙諾啡透皮貼劑對老年股骨轉(zhuǎn)子間骨折術(shù)后有更好的鎮(zhèn)痛效果,但術(shù)后2日內(nèi)注意藥物不良反應(yīng)的發(fā)生。
[Abstract]:Objective to observe the effect of buprenorphine transdermal patch on pain relief after intertrochanteric fracture in the elderly. Methods 67 elderly patients with intertrochanteric fracture were divided into trial group (n = 34) and control group (n = 33). The experimental group was treated with buprenorphine transdermal patch and the control group with the same package of blank patch without buprenorphine. Systolic blood pressure, diastolic blood pressure, heart rate and visual analogue score (VAS),) were recorded on 7 days after operation. Results the VAS of the experimental group was (6.69 鹵1.83), (4.22 鹵1.56), (3.73 鹵2.20), (3.13 鹵1.58) and the VAS of the control group was (8.15 鹵1.36), (7.75 鹵2.26), (7.14 鹵1.67), (6.82 鹵2.24) 5 days after treatment. There was significant difference between the two groups (P0.05). The main adverse drug reactions in the trial group were nausea and vomiting, rash, the main adverse drug reactions in the control group were nausea and vomiting, the incidence of adverse drug reactions were 23.53% and 6.06% respectively, the difference was statistically significant (P0.05). Conclusion the results showed that buprenorphine transdermal patch had better analgesic effect in elderly patients with intertrochanteric fracture, but attention should be paid to the occurrence of adverse drug reactions within 2 days after operation.
