【摘要】：[目的]總結(jié)應(yīng)用ACDF聯(lián)合術(shù)中顱骨牽引助推撬撥技術(shù)治療下頸椎單節(jié)段關(guān)節(jié)突絞鎖的臨床效果。[方法]采用ACDF聯(lián)合術(shù)中顱骨牽引助推撬撥技術(shù)治療30例單節(jié)段下頸椎關(guān)節(jié)突絞鎖患者,記錄關(guān)節(jié)突關(guān)節(jié)復(fù)位所需時間、手術(shù)時間、出血量等,并定期復(fù)查X線片及CT,通過測量椎間隙高度及Bridwell椎間融合標(biāo)準(zhǔn)評估椎間植骨融合情況,根據(jù)ASIS評級分析手術(shù)前后神經(jīng)功能,根據(jù)JOA評分評估脊髓功能恢復(fù)情況。[結(jié)果]30例患者均手術(shù)順利,關(guān)節(jié)突關(guān)節(jié)復(fù)位所需時間(2±1)min,手術(shù)時間(45±8)min,術(shù)中失血(30±5)ml。術(shù)后影像檢查顯示傷段100%復(fù)位,融合良好,椎體間隙高度增加。除3例患者外,其他患者術(shù)后1個月時ASIA評級顯著改善。與術(shù)前相比較,術(shù)后3個月JOA評分明顯改善。[結(jié)論]應(yīng)用ACDF聯(lián)合術(shù)中顱骨牽引助推撬撥技術(shù)能夠在短時間內(nèi)使關(guān)節(jié)突絞鎖復(fù)位,挽救脊髓功能,長期可保持椎間高度和脊柱正常序列,椎間融合可靠,操作簡單。
[Abstract]:Objective: to summarize the clinical effect of ACDF combined with skull traction and prying technique in the treatment of lower cervical spine single segment articular process strangulation. [methods] 30 patients with single segment lower cervical spine process strangulation were treated with ACDF combined with cranial traction and prying technique. The time required for reduction of articular process joint, operative time, and blood loss were recorded. X-ray films and CT, were used to evaluate the intervertebral fusion by measuring the height of intervertebral space and the standard of Bridwell intervertebral fusion. The neurological function before and after operation was analyzed according to ASIS rating and the recovery of spinal cord function was evaluated according to JOA score. [results] all the 30 patients underwent successful operation, and the time of reduction of articular facet joint was (2 鹵1) min, operation time (45 鹵8) min, operation blood loss was (30 鹵5) ml.. Postoperative imaging showed 100% reduction, good fusion and increased height of vertebral space. With the exception of 3 patients, the ASIA rating improved significantly 1 month after operation. Compared with preoperative, JOA scores were significantly improved 3 months after operation. [conclusion] the technique of ACDF combined with skull traction and prying during operation can reduce the articular process strangulation in a short time, save the spinal cord function, maintain the intervertebral height and the normal sequence of the spine for a long time, the intervertebral fusion is reliable and the operation is simple.
