【摘要】：目的比較InterTan髓內(nèi)釘和股骨近端抗旋髓內(nèi)釘(PFNA)內(nèi)固定治療老年不穩(wěn)定型股骨粗隆間骨折圍術(shù)期失血量。方法 96例老年不穩(wěn)定型股骨粗隆間骨折患者隨機分為兩組各48例,InterTan組采取InterTan髓內(nèi)釘內(nèi)固定,PFNA組采取PFNA內(nèi)固定,比較兩組總失血量、術(shù)中失血量、隱性失血量、顯性失血量。結(jié)果 InterTan組術(shù)中失血量(179±56.5)mL、顯性失血量(340.5±86.8)mL、隱性失血量(490.7±93.1)mL、總失血量(831.2±132.8)mL,PFNA組分別為(133±39.3)、(251.8±67.5)、(463.3±88.5)、(715.1±108.8)mL,兩組術(shù)中失血量、顯性失血量、總失血量比較,P均0.05。結(jié)論與PFNA內(nèi)固定比較,InterTan髓內(nèi)釘內(nèi)固定治療此類患者術(shù)中失血量、顯性失血量、總失血量多;兩種內(nèi)固定方式的隱性失血量雖無明顯差異,但隱性失血量在總失血量中占有較大比重,臨床上要引起重視,并采取有效措施。
[Abstract]:Objective to compare the perioperative blood loss with InterTan intramedullary nail (InterTan) and proximal femoral anti-circumflex intramedullary nail (PFNA) in the treatment of unstable intertrochanteric fractures in the elderly. Methods 96 elderly patients with unstable femoral intertrochanteric fracture were randomly divided into two groups, 48 patients in each group were treated with InterTan intramedullary nail fixation and PFNA internal fixation. The total blood loss, intraoperative blood loss, recessive blood loss and dominant blood loss were compared between the two groups. Results the intraoperative blood loss (179 鹵56.5) mL, dominant loss (340.5 鹵86.8) mL, recessive blood loss (490.7 鹵93.1) mL, total blood loss in InterTan group (831.2 鹵132.8) mL,PFNA group was (133 鹵39.3), (251.8 鹵67.5), (463.3 鹵88.5), (715.1 鹵108.8 mL, intraoperative blood loss. Conclusion compared with PFNA internal fixation, InterTan intramedullary nail internal fixation has no significant difference in the amount of blood loss during the operation, the dominant amount of blood loss and the total amount of blood loss, although there is no significant difference between the two internal fixation methods in the amount of recessive blood loss, there is no significant difference between the two internal fixation methods. However, recessive blood loss accounts for a large proportion of the total blood loss, so it should be paid more attention in clinic and effective measures should be taken.
