【摘要】：目的比較冠狀動脈旁路移植術(shù)中分段切開與全程切開獲取大隱靜脈近期臨床療效。方法納入溫州醫(yī)科大學(xué)附屬第一醫(yī)院2013年1月至2015年12月行冠狀動脈旁路移植術(shù)患者資料,進(jìn)行回顧性研究。據(jù)術(shù)中大隱靜脈獲取方式分為2組,全程切開組99例,分段切開組114例。收集2組患者的術(shù)前臨床資料,并對術(shù)中大隱靜脈獲取時間、獲取長度、獲取質(zhì)量及早期臨床結(jié)果、下肢切口并發(fā)癥情況進(jìn)行比較。術(shù)后1年應(yīng)用冠脈CTA檢查,比較靜脈橋血管通暢率。結(jié)果 2組患者臨床基礎(chǔ)資料無明顯統(tǒng)計(jì)學(xué)差異(P0.05),分段切開組下肢切口并發(fā)癥發(fā)生率較全程切開組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2組術(shù)后1年冠脈CTA檢查靜脈橋血管通暢率差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論分段切開獲取大隱靜脈技術(shù)安全可行,在降低術(shù)后下肢切口并發(fā)癥方面有優(yōu)勢,對術(shù)后近期通暢率無明顯影響,是臨床大隱靜脈取材較為經(jīng)濟(jì)和微創(chuàng)的方法。
[Abstract]:Objective to compare the short-term clinical effects of segmental incision and complete incision of great saphenous vein in coronary artery bypass grafting (CABG). Methods the data of patients undergoing coronary artery bypass grafting (CABG) from January 2013 to December 2015 in the first affiliated Hospital of Wenzhou Medical University were retrospectively studied. According to the method of obtaining the great saphenous vein, 99 cases in the whole incision group and 114 cases in the segmental incision group were divided into two groups. The preoperative clinical data of the two groups were collected, and the time, length, quality and early clinical results of the great saphenous vein during operation were compared, and the complications of lower extremity incision were compared. 1 year after operation, coronary CTA was used to compare the patency rate of venous grafts. Results there was no significant difference in the clinical basic data between the two groups (P0.05). The incidence of lower limb incision complications in the segmental incision group was lower than that in the whole incision group. The difference was statistically significant (P0.05). 2. 1 year after operation, there was no significant difference in the patency rate of venous grafts in CTA examination of coronary artery (P0.05). Conclusion it is safe and feasible to obtain great saphenous vein by segmental incision. It has advantages in reducing the complications of lower extremity incision and has no obvious effect on the patency rate in the short term. It is an economical and minimally invasive method for clinical use of great saphenous vein.
