【摘要】：目的:探討非壓迫性椎間盤(pán)髓核突出中自身免疫反應(yīng)對(duì)神經(jīng)根損傷的作用機(jī)制及臨床意義。方法:雌性SD大鼠48只,隨機(jī)分成模型組、對(duì)照組,每組24只。造模后10 d、20 d、40 d,每組隨機(jī)抽取8只,測(cè)定左后肢痛閾;檢測(cè)動(dòng)脈血中CD4+、CD8+T細(xì)胞比例及血清中TNF-α表達(dá);HE染色觀察神經(jīng)根形態(tài)學(xué)變化。結(jié)果:模型組在10 d、20 d時(shí),左后肢痛閾、CD4+、CD8+T細(xì)胞比例及TNF-α表達(dá)與對(duì)照組比較,差異均有統(tǒng)計(jì)學(xué)意義,在40 d時(shí)兩組比較,差異均無(wú)統(tǒng)計(jì)學(xué)差異。模型組10 d時(shí)腰神經(jīng)根橫斷面髓鞘部分崩解,呈空泡變;20 d時(shí)最嚴(yán)重,40 d時(shí)基本恢復(fù)正常;對(duì)照組未見(jiàn)異常。結(jié)論:在非壓迫性椎間盤(pán)髓核突出早期,T細(xì)胞介導(dǎo)的自身免疫及TNF-α對(duì)神經(jīng)根損傷發(fā)揮重要作用。
[Abstract]:Objective: to investigate the mechanism and clinical significance of autoimmune response to nerve root injury in non-compression protrusion of nucleus pulposus. Methods: 48 female SD rats were randomly divided into model group and control group with 24 rats in each group. After 10 days, 20 days and 40 days, 8 rats in each group were randomly selected to measure the pain threshold of left hind limb, and the ratio of CD4 and CD8 T cells in arterial blood and the expression of TNF- 偽 in serum were detected by HE staining to observe the morphological changes of nerve root. Results: compared with the control group, the ratio of CD4 / CD8 T cells and the expression of TNF- 偽 in the model group were significantly different from those in the control group at 10 days and 20 days, but there was no significant difference between the two groups at 40 days. In the model group, the myelin sheath of the lumbar nerve root on the transverse section of the lumbar nerve root disintegrated on the 10th day, and the most serious one returned to normal at the 20th day after vacuolation, but there was no abnormality in the control group. Conclusion: autoimmunity mediated by T cells and TNF- 偽 may play an important role in nerve root injury in the early stage of noncompressed disc nucleus pulposus herniation.
