【摘要】：目的探討一期治療外傷性膝關(guān)節(jié)脫位合并多韌帶損傷的近期療效。方法2012年3月-2013年12月,共一期關(guān)節(jié)鏡輔助下治療19例外傷性膝關(guān)節(jié)脫位合并多韌帶損傷的患者。其中男13例,女6例,年齡27-61歲(平均43歲),其中右膝10例,左膝9例,均為單膝。于傷后6h-2天入院,平均13h。19例患者(19膝)術(shù)中關(guān)節(jié)鏡探查均證實(shí)ACL、PCL同時(shí)斷裂;其中伴內(nèi)側(cè)副韌帶損傷(III度)11例,合并PLC損傷5例,外側(cè)半月板撕裂5例,內(nèi)側(cè)半月板撕裂3例;1膝患者同時(shí)伴髕骨脫位,MPFL急性斷裂。19膝中均同時(shí)應(yīng)用4股乆繩肌腱單束重建PCL、ACL,合并的11例MCL予以修復(fù),5例LCL予以修復(fù),以上韌帶均采用自體肌腱進(jìn)行重建。結(jié)果所有19例患者均獲13-24月的隨訪,平均18月。術(shù)后切口均I期愈合,無感染等并發(fā)癥發(fā)生。術(shù)后12月患膝關(guān)節(jié)屈曲達(dá)125°-140°,平均(130.0±8.1)°,術(shù)前(96.5±6.0)°;膝關(guān)節(jié)Lysholm評分術(shù)前(21.4±6.2)分,術(shù)后(75.6±9.5)分;這兩項(xiàng)結(jié)果,術(shù)前、術(shù)后比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。根據(jù)Lysholm膝關(guān)節(jié)功能評分,優(yōu)4例,良12例,可3例,優(yōu)良率84.2%。結(jié)論一期治療外傷性膝關(guān)節(jié)脫位合并多韌帶損傷近期療效滿意。
[Abstract]:Objective to investigate the short-term effect of one-stage treatment for traumatic dislocation of knee with multiple ligaments injury. Methods from March 2012 to December 2013, 19 patients with traumatic dislocation of knee joint complicated with multiple ligament injury were treated with arthroscopy. There were 13 males and 6 females, aged 27-61 years (mean 43 years), including 10 cases of right knee and 9 cases of left knee, all of them were single knee. After 6h-2 was admitted to hospital on the day after injury, the mean number of 13h.19 patients (19 knees) was confirmed by arthroscopic exploration, 11 cases were accompanied with medial collateral ligament injury (III degree), 5 cases were complicated with PLC injury, 5 cases with lateral meniscus tear, 11 cases with medial collateral ligament injury (III degree), 5 cases with lateral meniscus tear. Three patients with medial meniscus tears and one knee with patellar dislocation were treated with 4 strands of human hamstring tendon to reconstruct LCL, and 11 cases of MCL were repaired with LCL. The above ligaments were reconstructed with autogenous tendon. Results all 19 patients were followed up for 13-24 months with an average of 18 months. All the incisions healed in the first stage without infection and other complications. The knee joint flexion reached 125 擄-140 擄(mean (130.0 鹵8.1) 擄, (96.5 鹵6.0) 擄before operation, and (21.4 鹵6.2) points before and (75.6 鹵9.5) after operation with Lysholm score. There was significant difference between the two results before and after operation (P0.05). According to Lysholm knee function score, excellent 4 cases, good 12 cases, fair 3 cases, excellent and good rate 84.2%. Conclusion one-stage treatment for traumatic dislocation of knee with multiple ligaments injury is satisfactory.
