【摘要】：目的 分析高強度聚焦超聲(high intensity focused ultrasound,HIFU)對肝包蟲病的臨床治療效果和治療過程的安全性,評估肝包蟲病的HIFU治療價值,指導臨床工作治療肝包蟲病。方法回顧性分析2008～2010年重慶醫(yī)科大學附二院院海扶中心明確診斷并經HIFU治療的8例肝包蟲患者的臨床資料,比較手術前、后患者癥狀、影像學結果變化并追蹤隨訪1年,了解肝包蟲病的治療結果及復發(fā)情況,評估HIFU對肝包蟲的治療效果；分析術后肝區(qū)疼痛、發(fā)熱、術區(qū)皮膚損害等并發(fā)癥并比較手術后患者肝功能指標變化情況,評估HIFU對肝包蟲治療的安全性。結果經HIFU治療后所有患者臨床癥狀明顯改善,影像學上囊腫較前明顯好轉,術后患者肝功能損害在5天內基本恢復正常,其中有1例患者術中出現(xiàn)嘔吐,術后未再發(fā)生嘔吐癥狀,3例患者出現(xiàn)2-6天的持續(xù)性低熱,6例患者出現(xiàn)2-7天的尚能忍受的肝區(qū)疼痛,3例患者出現(xiàn)皮膚腫脹,但所有并發(fā)癥均在短期內恢復。術后1年的隨訪過程中除1例患者術后4月發(fā)現(xiàn)肝包蟲復發(fā),再次HIFU治療好轉外其余患者均未見肝包蟲復發(fā)的癥狀及相應影像學變化。結論HIFU治療肝包蟲病安全有效,其具有手術創(chuàng)傷小,并發(fā)癥少等特點,值得臨床推廣。
[Abstract]:Objective to evaluate the clinical efficacy and safety of high intensity focused ultrasound (high intensity focused) ultrasound in the treatment of hepatic hydatid disease, to evaluate the value of HIFU in the treatment of hepatic hydatid disease and to guide clinical work in the treatment of hepatic hydatid disease. Methods the clinical data of 8 patients with hepatic hydatid were analyzed retrospectively from 2008 to 2010 in Haifu Center, second Hospital of Chongqing Medical University and treated with HIFU. The symptoms and imaging results of the patients before and after operation were compared and followed up for 1 year. To understand the treatment and recurrence of hepatic hydatid disease, to evaluate the efficacy of HIFU in the treatment of hepatic hydatid disease, to analyze the complications of postoperative liver pain, fever, skin damage and so on, and to compare the changes of liver function indexes after operation. To evaluate the safety of HIFU in the treatment of hepatic hydatid. Results after HIFU treatment, all the patients' clinical symptoms were obviously improved, and the cysts were obviously improved in imaging. The damage of liver function returned to normal in 5 days after operation, and one patient had vomiting during the operation. There was no recurrence of vomiting symptoms in 3 patients with persistent low fever for 2 to 6 days. Skin swelling was found in 3 patients who were still able to bear pain in liver area for 2 to 7 days, but all the complications recovered in a short period of time. The recurrence of hepatic hydatid was found in one patient at 4 months after operation, and no symptoms and imaging changes of hepatic hydatid recurrence were found in other patients after 1 year of follow-up. Conclusion HIFU is safe and effective in the treatment of hepatic hydatid disease.
