【摘要】：目的:探討一種設(shè)計(jì)簡(jiǎn)單并可以廣泛應(yīng)用的新方法矯正乳頭肥大。方法:收集從2015年3月到2016年9月期間廣西醫(yī)科大學(xué)第一附屬醫(yī)院整形美容外科門(mén)診的乳頭肥大患者共5例(10個(gè)乳頭),女性,平均年齡32歲。術(shù)前乳頭平均直徑為17.1mm。乳頭平均高度為18.1mm。采用脫套法(在乳頭頂面正中設(shè)計(jì)圓形切口,乳頭側(cè)面設(shè)計(jì)環(huán)形切口,從乳頭頂面垂直向下切開(kāi)乳頭皮膚,由皮下向乳頭基底脫套剝離,術(shù)中注意保留乳頭中央組織的血運(yùn)和乳腺中央導(dǎo)管的完整,然后在3、6、9、12點(diǎn)位置向乳頭基底部縱形切開(kāi)剝離下的乳頭皮膚套,分別向四個(gè)方向打開(kāi),沿著乳頭側(cè)面環(huán)形設(shè)計(jì)線剪除多余皮膚及皮下組織。環(huán)形縫合切口再造乳頭。)對(duì)肥大乳頭進(jìn)行矯正,術(shù)后門(mén)診及電話隨訪6個(gè)月,觀察乳頭有無(wú)壞死,評(píng)估乳頭的形狀、大小、感覺(jué)功能及疤痕情況。結(jié)果:5例共10個(gè)乳頭,術(shù)后隨訪6個(gè)月均滿意,沒(méi)有出現(xiàn)乳頭壞死、乳頭感染;乳頭橫徑為平均數(shù)為9.2mm,乳頭高度平均數(shù)為5.6mm;肥大乳頭的直徑和高度明顯變小,外觀改善,疤痕不明顯,乳頭感覺(jué)良好。結(jié)論:脫套法乳頭縮小術(shù)設(shè)計(jì)簡(jiǎn)單、手術(shù)容易操作、效果肯定,可廣泛應(yīng)用于乳頭肥大矯正。
[Abstract]:Objective: to explore a new method for correcting nipple hypertrophy with simple design and wide application. Methods: from March 2015 to September 2016, a total of 5 cases (10 nipples) with female nipple hypertrophy, mean age 32 years, were collected from the outpatient department of plastic and cosmetic surgery, first affiliated Hospital of Guangxi Medical University. The mean diameter of nipple before operation was 17.1 mm. The average height of nipple was 18.1mm. A circular incision was designed in the middle of the top of the nipple, a circular incision was designed on the side of the nipple, the skin of the papilla was cut vertically down from the top of the nipple, and the skin was removed from the subcutaneous to the base of the nipple. During the operation, the blood flow of the central tissue of the papilla and the integrity of the central duct of the mammary gland were preserved, and then the skin sleeve of the nipple was cut into the base of the nipple at 12 o'clock and opened in four directions respectively. Cut off excess skin and subcutaneous tissue along the circular design line of the side of the nipple. Ring suture incision to reconstruct nipple. The mast nipples were corrected and followed up by telephone and outpatient for 6 months. The nipple necrosis was observed and the shape, size, sensory function and scar of the nipple were evaluated. Results the 10 nipples of 5 cases were all satisfactory for 6 months, no nipple necrosis, nipple infection, the average diameter of nipple was 9.2 mm, the average height of nipple was 5.6 mm, the diameter and height of mast nipple were obviously smaller and the appearance was improved. The scar is not obvious, the nipple feels good. Conclusion: the method of nipple reduction is simple in design, easy to operate and effective, and can be widely used in the correction of nipple hypertrophy.
