【摘要】：目的總結(jié)內(nèi)髁Hoffa骨折合并前交叉韌帶斷裂的診療經(jīng)驗(yàn)。方法 2012年1月—2016年2月,收治3例外傷致內(nèi)髁Hoffa骨折合并前交叉韌帶斷裂患者。男2例,女1例;年齡分別為59、43、55歲。傷后至入院時(shí)間為2、3、1 h。采用空心螺釘固定骨折塊,關(guān)節(jié)鏡下一期前交叉韌帶重建治療。結(jié)果術(shù)后患者切口均Ⅰ期愈合。3例患者分別獲隨訪10、9、12個(gè)月。X線片復(fù)查示,骨折均愈合,愈合時(shí)間分別為12、14、16周;隨訪期間無創(chuàng)傷性關(guān)節(jié)炎發(fā)生。末次隨訪時(shí),參照Letenneur提出的Hoff骨折術(shù)后功能恢復(fù)評價(jià)系統(tǒng),膝關(guān)節(jié)功能均達(dá)優(yōu)。結(jié)論對于內(nèi)髁Hoffa骨折合并前交叉韌帶斷裂,切開復(fù)位空心螺釘內(nèi)固定聯(lián)合膝關(guān)節(jié)鏡下前交叉韌帶重建術(shù)是一種有效、安全方法。
[Abstract]:Objective to summarize the experience of diagnosis and treatment of medial condylar Hoffa fracture with anterior cruciate ligament fracture. Methods from January 2012 to February 2016, three patients with Hoffa fracture of medial condyle associated with anterior cruciate ligament fracture were treated. There were 2 males and 1 females, aged 59 / 43 and 55 years old respectively. The time from injury to admission was 2 minutes and 3 minutes. First stage anterior cruciate ligament reconstruction under arthroscopy was performed with hollow screw fixation. Results Thirty three patients with primary wound healing were followed up for 10 ~ 9 months and 12 months. X-ray examination showed that the fracture healed for 12 ~ 14 ~ 16 weeks, and no traumatic arthritis occurred during the follow-up period. At the last follow-up, the function of knee joint was excellent according to the evaluation system of postoperative function of Hoff fracture proposed by Letenneur. Conclusion for Hoffa fracture of medial condyle with anterior cruciate ligament rupture, open reduction and hollow screw fixation combined with anterior cruciate ligament reconstruction under knee arthroscopy is an effective and safe method.
