【摘要】：目的:本研究旨在觀察利用中藥內(nèi)服配合持續(xù)被動(dòng)訓(xùn)練機(jī)(CPM機(jī))功能鍛煉在脛骨平臺(tái)骨折術(shù)后促進(jìn)骨折愈合方面和改善膝關(guān)節(jié)功能方面的臨床療效。方法:本研究所有病例均選自2015年3月至2016年3月期間沈陽(yáng)市骨科醫(yī)院的單側(cè)脛骨平臺(tái)骨折住院患者,全部接受切開(kāi)復(fù)位鎖定鋼板螺釘內(nèi)固定手術(shù)治療,共46例,隨機(jī)分為2組。治療組23例在術(shù)后采用中藥內(nèi)服聯(lián)合CPM功能鍛煉,對(duì)照組23例在術(shù)后只采用CPM功能鍛煉。按照《外科學(xué)》中的骨折臨床愈合標(biāo)準(zhǔn)評(píng)估骨折愈合情況,記錄骨折臨床愈合時(shí)間。術(shù)后3個(gè)月,術(shù)后6個(gè)月依據(jù)HSS膝關(guān)節(jié)評(píng)分標(biāo)準(zhǔn)評(píng)價(jià)患膝功能恢復(fù)情況。結(jié)果:共計(jì)46例脛骨平臺(tái)患者手術(shù)后有6個(gè)月的隨訪期,患肢均達(dá)到臨床愈合標(biāo)準(zhǔn),治療組骨折臨床愈合時(shí)間(8.65±1.40)周,對(duì)照組骨折臨床愈合時(shí)間(9.70±1.50)周。術(shù)后3個(gè)月治療組HSS功能評(píng)分(45.04±5.50),對(duì)照組HSS功能評(píng)分(41.61±5.32)。術(shù)后6個(gè)月治療組HSS功能評(píng)分(89.70±7.84),對(duì)照組HSS功能評(píng)分(79.65±8.20)兩組觀察并比較骨折臨床愈合時(shí)間、HSS功能評(píng)分,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:對(duì)于接受切開(kāi)復(fù)位鎖定鋼板螺釘內(nèi)固定手術(shù)治療的脛骨平臺(tái)骨折患者,手術(shù)后在CPM機(jī)功能鍛煉的基礎(chǔ)上采用中醫(yī)藥聯(lián)合治療,利于骨折愈合及患膝功能的恢復(fù),療效優(yōu)于只采用CPM機(jī)功能鍛煉者,值得臨床上骨折術(shù)后推廣中醫(yī)藥聯(lián)合治療。
[Abstract]:Objective: to observe the clinical effect of traditional Chinese medicine (TCM) combined with continuous passive training machine (CPM) in promoting fracture healing and improving knee joint function after tibial plateau fracture. Methods: from March 2015 to March 2016, 46 patients with unilateral tibial plateau fractures were treated with open reduction and locking plate screw fixation. They were randomly divided into two groups. The treatment group (23 cases) were treated with traditional Chinese medicine combined with CPM function exercise after operation, while the control group (23 cases) only took CPM function exercise after operation. The fracture healing was evaluated according to the criteria of fracture healing in surgery and the time of fracture healing was recorded. After 3 months and 6 months after operation, the functional recovery of the affected knee was evaluated according to the HSS knee scoring standard. Results: a total of 46 patients with tibial plateau were followed up for 6 months. The clinical healing time of the affected limbs was (8.65 鹵1.40) weeks in the treatment group and (9.70 鹵1.50) weeks in the control group. The HSS function score of the treatment group was (45.04 鹵5.50) months after operation, and the HSS function score of the control group was (41.61 鹵5.32) months after operation. Six months after operation, the scores of HSS function in the treatment group (89.70 鹵7.84) and the control group (79.65 鹵8.20) were observed and compared. The difference was statistically significant (P0.05). Conclusion: for the patients with tibial plateau fracture treated by open reduction and locking plate and screw internal fixation, the combined therapy of traditional Chinese medicine on the basis of CPM machine function exercise after operation is beneficial to fracture healing and recovery of knee function. The curative effect is better than only using CPM machine function exercise, it is worth popularizing traditional Chinese medicine combination treatment after operation of fracture.
