【摘要】：目的分析術(shù)前外周血中性粒細(xì)胞與淋巴細(xì)胞計(jì)數(shù)比(NLR)在甲狀腺良惡性結(jié)節(jié)鑒別診斷中的價(jià)值。方法回顧性分析2014-07-01—2015-06-30于中國(guó)人民解放軍總醫(yī)院普通外科經(jīng)手術(shù)治療的485例甲狀腺結(jié)節(jié)病人的臨床資料,根據(jù)術(shù)后常規(guī)病理學(xué)檢查結(jié)果分為良性組和惡性組,對(duì)每組病例所對(duì)應(yīng)的NLR進(jìn)行對(duì)比分析,應(yīng)用ROC曲線分析鑒別甲狀腺良惡性結(jié)節(jié)的最佳cut-off值。結(jié)果甲狀腺結(jié)節(jié)良性組和惡性組的NLR值分別為(1.6±0.5)和(1.9±1.0),惡性組有較高的中性粒細(xì)胞計(jì)數(shù)及較低的淋巴細(xì)胞計(jì)數(shù),兩組差異有統(tǒng)計(jì)學(xué)意義(P0.05)。由ROC曲線分析得出鑒別甲狀腺結(jié)節(jié)良惡性的NLR最佳cut-off值為1.77,敏感度為65.8%,特異度為79.7%,AUC為0.723。結(jié)論術(shù)前外周血NLR值可指導(dǎo)甲狀腺良惡性結(jié)節(jié)的鑒別診斷,高NLR值預(yù)示結(jié)節(jié)惡性。
[Abstract]:Objective to analyze the value of (NLR) in differential diagnosis of benign and malignant thyroid nodules. Methods the clinical data of 485 patients with thyroid nodule treated in general surgery department of Chinese people's Liberation Army General Hospital were retrospectively analyzed. The patients were divided into benign group and malignant group according to the results of postoperative routine pathological examination. The NLR of each group was compared and analyzed. The best cut-off value of ROC curve was used to distinguish benign and malignant thyroid nodules. Results the NLR values of benign and malignant thyroid nodule group were (1.6 鹵0.5) and (1.9 鹵1.0), respectively. The neutrophil count and lymphocyte count in malignant group were higher than those in malignant group (P0.05). According to ROC curve analysis, the best cut-off value of NLR for differentiating benign and malignant thyroid nodules was 1.77, the sensitivity was 65.8, the specificity was 79.7% and the specificity was 0.723. Conclusion preoperative NLR value of peripheral blood can guide the differential diagnosis of benign and malignant thyroid nodules, and high NLR value can predict the malignancy of thyroid nodules.
