【摘要】：目的采用腔內(nèi)修復(fù)術(shù)治療外周血管創(chuàng)傷患者,觀察其中遠(yuǎn)期療效。方法回顧性分析我中心2008年3月至2016年4月46例外周血管創(chuàng)傷患者的臨床資料,其中男性34例,女性12例;年齡23~61(39.8±11.6)歲。損傷類型:動(dòng)脈內(nèi)膜損傷16例,動(dòng)脈部分?jǐn)嗔?1例,動(dòng)脈完全斷裂7例,假性動(dòng)脈瘤9例,動(dòng)靜脈瘺3例。46例均經(jīng)血管造影明確診斷,同時(shí)行腔內(nèi)修復(fù)治療。分別在術(shù)后3、6、12個(gè)月進(jìn)行隨訪,之后每年隨訪1次。結(jié)果 46例患者均成功完成手術(shù),手術(shù)成功率100%。手術(shù)時(shí)間30~90(50±16)min。腔內(nèi)修復(fù)治療后受損動(dòng)脈血流恢復(fù)通暢,假性動(dòng)脈瘤及動(dòng)靜脈瘺消失,無(wú)對(duì)比劑外溢現(xiàn)象。無(wú)圍手術(shù)期死亡及肢體缺血壞死、感染、截肢等嚴(yán)重并發(fā)癥。46例患者無(wú)失訪,隨訪12~61個(gè)月,中位隨訪時(shí)間43.5個(gè)月。1例出現(xiàn)支架內(nèi)血栓形成,予溶栓治療后血流恢復(fù);1例支架內(nèi)再狹窄50%,予球囊擴(kuò)張等處理后血流恢復(fù)通暢;其余患者動(dòng)脈血流保持通暢。隨訪期間無(wú)支架移位、斷裂、內(nèi)漏等并發(fā)癥發(fā)生。結(jié)論腔內(nèi)修復(fù)術(shù)治療外周血管創(chuàng)傷中遠(yuǎn)期療效顯著。
[Abstract]:Objective to observe the long-term effect of endovascular repair for peripheral vascular trauma. Methods the clinical data of 46 patients with peripheral vascular trauma from March 2008 to April 2016 were retrospectively analyzed, including 34 males and 12 females, aged 2361 (39.8 鹵11.6) years. There were 16 cases of arterial intimal injury, 11 cases of partial rupture of artery, 7 cases of complete rupture of artery, 9 cases of pseudoaneurysm and 3 cases of arteriovenous fistula. They were followed-up 3 and 12 months after operation, and then followed up once a year. Results all 46 patients completed the operation successfully, and the success rate was 100%. The operative time was 30 ~ 90 (50 鹵16) min. After endovascular repair, the blood flow of injured artery recovered, pseudoaneurysm and arteriovenous fistula disappeared, and there was no contrast overflow. There were no serious complications such as perioperative death, limb necrosis, infection, amputation and other serious complications. The follow-up period ranged from 12 to 61 months. The median follow-up time was 43.5 months. After thrombolytic therapy, the blood flow recovered in 1 case with restenosis in stents, and after balloon dilatation, the blood flow recovered unobstructed, and the arterial blood flow in other patients remained unobstructed. There were no complications such as stent displacement, rupture and internal leakage during follow-up. Conclusion Endovascular repair is effective in the treatment of peripheral vascular trauma.
