【摘要】：目的總結(jié)旋股外側(cè)動(dòng)脈降支嵌合穿支皮瓣游離移植治療脛骨創(chuàng)傷后骨髓炎的臨床經(jīng)驗(yàn)。方法2014年8月-2015年9月,8例脛骨創(chuàng)傷后骨髓炎在徹底清創(chuàng)后采用旋股外側(cè)動(dòng)脈降支嵌合穿支皮瓣游離移植修復(fù),股外側(cè)肌瓣填塞脛骨死腔,穿支皮瓣覆蓋淺表創(chuàng)面。結(jié)果 8例皮瓣全部順利成活,其中,1例術(shù)后感染經(jīng)換藥治療愈合,其余7例創(chuàng)口一期愈合。術(shù)后隨訪6~18個(gè)月,脛骨骨髓炎無(wú)復(fù)發(fā),皮瓣色澤、質(zhì)地好,皮瓣供區(qū)直接縫合,僅遺留線性瘢痕。結(jié)論旋股外側(cè)動(dòng)脈降支嵌合穿支皮瓣肌瓣血運(yùn)好且與皮瓣僅以穿支血管相連擁有足夠自由度,可以實(shí)現(xiàn)創(chuàng)面立體修復(fù),是一種治療脛骨創(chuàng)傷后骨髓炎的有效方法。
[Abstract]:Objective to summarize the clinical experience of free transplantation of descending branch of lateral circumflex femoral artery with perforating branch flap in the treatment of post-traumatic osteomyelitis of tibia. Methods from August 2014 to September 2015, 8 cases of post-traumatic osteomyelitis of tibia were repaired by free transplantation of chimeric perforator flap of descending branch of lateral femoral circumflex artery after thorough debridement, the dead cavity of tibia was filled with lateral femoral muscle flap, and superficial wound was covered by perforating branch flap. Results all the skin flaps survived successfully. One case of infection healed after operation and the other 7 cases healed at the first stage. Follow up for 6 ~ 18 months showed no recurrence of tibial osteomyelitis, good color and texture of the flap, direct suture of the donor area of the flap, and only linear scar. Conclusion the interlocking perforator flap with descending branch of lateral circumflex femoral artery has enough freedom of freedom and it is an effective method for the treatment of osteomyelitis after trauma of tibia.
