【摘要】：目的探討自體脾片移植術(shù)在脾外傷患者中的臨床應用價值。方法收集脾外傷患者120例,其中自體脾片移植組72例,切脾組48例,比較兩組手術(shù)時間、術(shù)中出血、術(shù)后住院天數(shù)、術(shù)后并發(fā)癥發(fā)生率及術(shù)前和術(shù)后不同時期免疫指標的差異。結(jié)果自體脾片移植組手術(shù)時間長于切脾組(P0.05),術(shù)中出血、術(shù)后住院天數(shù)及術(shù)后并發(fā)癥發(fā)生率與切脾組相比差異無統(tǒng)計學意義。術(shù)后1 d兩組各免疫指標均較術(shù)前明顯降低(P0.05)。術(shù)后1周兩組各免疫指標均較術(shù)后1 d升高(P0.05)。術(shù)后3個月自體脾片移植組各免疫指標恢復至術(shù)前水平(P0.05),且各免疫指標均優(yōu)于切脾組(P0.05)。結(jié)論脾切除能使機體免疫功能急劇下降,但機體免疫功能短期內(nèi)可回升至一定水平,行自體脾片移植能使機體免疫功能恢復至術(shù)前水平。
[Abstract]:Objective to investigate the clinical value of autologous splenic graft in the patients with splenic trauma. Methods 120 cases of splenic trauma were collected, including 72 cases of splenic autograft group and 48 cases of splenectomy group, compared two groups of operation time, intraoperative bleeding, postoperative hospitalization days, postoperative complications and the difference of immune indexes before and after the operation. The operation time of the autologous splenic graft group was longer than that of the splenectomy group (P0.05). The intraoperative bleeding, postoperative hospital stay and the incidence of postoperative complications were not statistically significant compared with those in the splenectomy group. The immune indexes of the 1 D two groups after operation were significantly lower than those before the operation (P0.05). 1 weeks after the operation, the two groups were all higher than 1 d after operation (P0.05). 3 months postoperatively autologous The immune indexes of the splenic graft group were recovered to the preoperative level (P0.05), and all the immune indexes were superior to the splenectomy group (P0.05). Conclusion splenectomy can reduce the immune function sharply, but the immune function of the body can rise to a certain level in the short term, and the immune function of the body can be restored to the preoperative level by the autotransplantation of the spleen.
