【摘要】：目的探討經(jīng)皮椎體成形術(shù)(PVP)聯(lián)合射頻消融術(shù)(RFA)對骨質(zhì)疏松性壓縮性脊柱骨折患者術(shù)后疼痛及生活質(zhì)量的影響。方法選取骨質(zhì)疏松性脊柱壓縮性骨折患者77例,按治療方法分為對照組35例、觀察組42例。對照組單純采用PVP,觀察組在PVP基礎(chǔ)上聯(lián)合RFA。記錄術(shù)中肺栓塞、骨水泥滲漏等并發(fā)癥情況。術(shù)后1個(gè)月評價(jià)臨床療效。術(shù)前、術(shù)后l周、3個(gè)月、6個(gè)月及12個(gè)月時(shí)采用目測類比(VAS)評分及Barthel指數(shù)進(jìn)行疼痛及生活質(zhì)量評分。結(jié)果對照組、觀察組有效率分別為91.43%、92.86%,兩組有效率差異無統(tǒng)計(jì)學(xué)意義(P0.05)。與治療前比較,兩組治療后VAS評分均下降、Barthel指數(shù)均升高(P均0.05)。觀察組術(shù)后3個(gè)月、術(shù)后6個(gè)月及術(shù)后12個(gè)月時(shí)VAS評分均低于對照組,Barthel指數(shù)均高于對照組(P均0.05)。兩組術(shù)中均無肺栓塞等嚴(yán)重并發(fā)癥發(fā)生。結(jié)論 PVP聯(lián)合RFA治療骨質(zhì)疏松性脊柱壓縮性骨折可明顯緩解患者術(shù)后疼痛,提高生活質(zhì)量。
[Abstract]:Objective to investigate the effect of percutaneous vertebroplasty (PVP) combined with radiofrequency ablation (RFA) on postoperative pain and quality of life (QOL) in patients with osteoporotic vertebral compression fracture. Methods Seventy-seven patients with osteoporotic spinal compression fracture were divided into control group (n = 35) and observation group (n = 42). The control group was treated with PVP alone, and the observation group was combined with RFA on the basis of PVP. The complications of pulmonary embolism and bone cement leakage were recorded. The clinical effect was evaluated 1 month after operation. Preoperative, 1 week, 3 months, 6 months and 12 months after operation, pain and quality of life scores were evaluated by visual analogy (VAS) score and Barthel index. Results in the control group, the effective rate of the observation group was 91.43 and 92.86, respectively. There was no significant difference between the two groups in the effective rate (P0.05). Compared with before treatment, VAS scores decreased and Barthel index increased after treatment in both groups (P < 0. 05). The VAS score in the observation group was lower than that in the control group at 3 months, 6 months and 12 months after operation (P < 0. 05). No serious complications such as pulmonary embolism occurred in both groups. Conclusion PVP combined with RFA can significantly relieve postoperative pain and improve quality of life in patients with osteoporotic spinal compression fracture.
