【摘要】：研究目的分析Pilon骨折的病因及受傷機(jī)制,比較切開復(fù)位有限內(nèi)固定結(jié)合外固定架手術(shù)治療方法相對傳統(tǒng)單純內(nèi)固定手術(shù)治療方式的優(yōu)劣,通過比較結(jié)果為臨床治療Pilon骨折手術(shù)方式提供可更加有效的治療方案。研究方法選取于我科在2014年9月-2016年6月收治的62例Pilon骨折患者,根據(jù)選擇不同的手術(shù)方式將總體病人樣本分為觀察組和對照組兩組。對觀察組患者給予行脛骨遠(yuǎn)端切開復(fù)位有限內(nèi)固定結(jié)合外固定手術(shù)予以治療,對照組行單純切開復(fù)位內(nèi)固定治療。記錄兩組患者手術(shù)術(shù)后第3個月及第12個月的踝關(guān)節(jié)Mazur評分等多項指標(biāo)并分析比較。結(jié)果兩組患者分別在術(shù)后第三個月及術(shù)后第十二個月的踝關(guān)節(jié)功能Mazur評分的差異具有統(tǒng)計學(xué)意義(P0.05);兩組患者術(shù)后并發(fā)癥方面的差異同樣具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論在新鮮閉合Pilon骨折的患者手術(shù)治療方式的選擇方面,采用切開復(fù)位有限內(nèi)固定結(jié)合外固定架的手術(shù)方式治療的患者術(shù)后踝關(guān)節(jié)的功能恢復(fù)較佳,術(shù)后并發(fā)癥較單純內(nèi)固定治療而言相對較少,踝關(guān)節(jié)功能恢復(fù)情況較佳。
[Abstract]:Objective to analyze the etiology and injury mechanism of Pilon fracture and compare the advantages and disadvantages of open reduction and limited internal fixation combined with external fixation. The results provide a more effective method for the treatment of Pilon fracture. Methods 62 patients with Pilon fracture admitted in our department from September 2014 to June 2016 were divided into two groups: observation group and control group according to different surgical methods. The patients in the observation group were treated with open reduction and limited internal fixation of distal tibia combined with external fixation, while the patients in the control group were treated with simple open reduction and internal fixation. The Mazur scores of ankle joint in the 3rd and 12th month after operation were recorded and compared between the two groups. Results the Mazur scores of ankle function in the third month and the twelfth month after operation were significantly different between the two groups (P0.05), and the differences of postoperative complications between the two groups were also statistically significant (P0.05). Conclusion in the choice of surgical treatment for fresh closed Pilon fracture, the functional recovery of ankle joint in patients with open reduction and limited internal fixation combined with external fixation is better. The postoperative complications were less than that of internal fixation alone, and the recovery of ankle function was better.
