【摘要】：背景:腰椎小關(guān)節(jié)骨性關(guān)節(jié)炎(Lumbar facet joint osteoarthritis,LFJOA)是常見的腰椎退變性疾病,也是引起慢性腰痛的常見原因之一,尤其在老年人群中伴嚴(yán)重椎小關(guān)節(jié)退變更易引起下腰痛。然而,迄今為止椎小關(guān)節(jié)骨性關(guān)節(jié)炎(Facet joint osteoarthritis FJOA)比其他重要骨性關(guān)節(jié)炎(Osteoarthritis OA)得到的研究少得多,如膝關(guān)節(jié)骨性關(guān)節(jié)炎和其他脊柱病理解剖特性的疾病,如退行性椎間盤疾病。中國已經(jīng)進(jìn)入老齡化社會(huì),LFJOA發(fā)病人口急劇上升,且隨著經(jīng)濟(jì)的發(fā)展,人們追求更高的生活質(zhì)量,然而至今關(guān)于LFJOA源性慢性腰痛的診治尚存在諸多爭議。目的:通過綜述目前LFJOA引起慢性腰痛的診療方面的研究,從而為臨床上更好的診療該病提供一些參考。方法:以“腰椎,椎小關(guān)節(jié)骨性關(guān)節(jié)炎或關(guān)節(jié)突關(guān)節(jié)炎,慢性腰痛”等關(guān)鍵詞進(jìn)行檢索,找出LFJOA源性慢性腰痛的相關(guān)研究,并對其進(jìn)行分析總結(jié)。結(jié)果與結(jié)論:目前有大量關(guān)于LFJOA的文獻(xiàn),綜合這些研究結(jié)果表明:LFJOA是引起慢性腰痛的常見原因之一,但對其診斷尚無統(tǒng)一的標(biāo)準(zhǔn),治療方法也多種多樣,一般通過保守治療和介入治療大多數(shù)患者都能取得相對滿意的療效,只有少數(shù)退變嚴(yán)重和有明顯神經(jīng)癥狀且合并脊椎其它退行性變(如椎管狹窄、腰椎不穩(wěn)、椎間盤退變等)的患者可能考慮開放手術(shù)治療。
[Abstract]:Background: (Lumbar facet joint osteoarthritis (LFJOA) is a common degenerative disease of lumbar vertebrae and one of the common causes of chronic low back pain. However, to date, (Facet joint osteoarthritis FJOA) has been much less studied than other important osteoarthritis (Osteoarthritis OA), such as knee osteoarthritis and other diseases with pathological and anatomical characteristics of the spine, such as degenerative disc disease. China has entered an aging society, the incidence of LFJOA population has risen sharply, and with the development of economy, people seek a higher quality of life. However, the diagnosis and treatment of chronic low back pain caused by LFJOA is still controversial. Objective: to provide some references for clinical diagnosis and treatment of chronic low back pain caused by LFJOA. Methods: the key words of "lumbar vertebrae, facet osteoarthritis or arthritic process arthritis, chronic low back pain" were searched to find out the related research of chronic low back pain caused by LFJOA, and to analyze and summarize them. Results & conclusion: there are a lot of literatures about LFJOA. The results show that the LFJOA is one of the common causes of chronic low back pain. Most patients can obtain relatively satisfactory results through conservative treatment and interventional therapy. Only a few patients with severe degeneration and obvious neurological symptoms and other degenerative changes of the spine (such as spinal canal stenosis, lumbar instability, etc.) Patients with disc degeneration, etc., may consider open surgery.
