PDGF-BB在腦外傷合并骨折患者血液中的表達(dá)及臨床意義

【摘要】:目的:通過對腦外傷骨折患者傷后不同時(shí)間點(diǎn)血液中的PDGF-BB含量進(jìn)行測定,分析和探討其在骨折加速愈合過程中的作用。方法:于2015年1月~2016年8月,在筆者所在醫(yī)院骨一科、骨二科以及神經(jīng)外科住院患者中選取40例腦外傷合并骨折患者(A組)、40例單純腦外傷患者(B組)、40例單純骨折患者(C組)作為研究對象,在傷后第1天、第7天、第14天、第30天采集患者的空腹靜脈血-70℃保存。另選取同期在我院進(jìn)行健康體檢者的40例健康體檢者作為參照對象,將其設(shè)置為D組。采集受檢者的空腹靜脈血液,然后對這四組血液樣本進(jìn)行PDGF-BB測定。A組、C組患者于骨折手術(shù)后4周、8周、12周進(jìn)行X線片檢查,觀察骨痂的形成情況,經(jīng)統(tǒng)計(jì)學(xué)分析做出結(jié)論。結(jié)果:A、B、C三組在傷后第1天.第7天.第14天.第30天測的PDGF-BB表達(dá)水平分別為A組(239.03±23.13.293.05±30.73、369.14±45.67、222.59±31.80ρg/ml)、B組(231.07±18.89、289.37±33.83、354.45±40.98、203.01±29.98ρg/ml)、C組(60.38±9.21、72.86±12.83、104.43±18.82、68.32±12.36ρg/ml)D組(28.47±8.42ρg/ml)。D組受檢者血清中PDGF-BB表達(dá)呈低水平,顯著低于A、B、C三組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。A組、B組PDGF-BB表達(dá)在傷后第1天.第7天.第14天呈增高趨勢,第30天A組.B組PDGF-BB表達(dá)水平回落,其兩組PDGF-BB在傷后第1天、第7天、第14天、第30天表達(dá)水平比較無明顯差異(P0.05),且均高于C組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。骨折術(shù)后4周A組形成骨痂數(shù)(發(fā)生6例、未發(fā)生34例)C組(發(fā)生1例、未發(fā)生39例)差異有統(tǒng)計(jì)學(xué)意義(P0.05)。8周A組(發(fā)生29例、未發(fā)生11例)、C組(發(fā)生14例、未發(fā)生26例)、12周A組(發(fā)生39例、未發(fā)生1例)、C組(發(fā)生33例、未發(fā)生7例)差異有統(tǒng)計(jì)學(xué)意義(p0.05)結(jié)論:腦外傷后PDGF-BB在外周血的表達(dá)增高,其可能是加速骨折愈合的因子之一。
