【摘要】：目的： 觀察中藥封包配合常規(guī)抗炎藥物對股骨干骨折術后的恢復的治療效果。期望為臨床治療股骨干骨折患者術后的康復提供一種中西醫(yī)結(jié)合的思路及方法。 方法： 隨機選擇來自湖南常德市第一中醫(yī)院住院病人股骨干骨折患者共60例,根據(jù)患者住院時間與住院號先后順序隨機分為兩組,即治療組30例(中藥封包外敷),對照組30例(云南白藥膏外敷)。治療組患者股骨干骨折后均采用閉合彈性髓內(nèi)針內(nèi)固定手術,術后予以常規(guī)抗炎藥物三天,術后第二天開始在患者骨折斷端疼痛腫脹處予以中藥封包外用治療,30min/次,每天進行兩次治療(上午、下午各一次)。對照組患者骨折后亦同治療組采用相同方式手術,術后予以常規(guī)抗炎藥物三天,術后第二天在患者骨折斷端疼痛腫脹處予以云南白藥膏貼敷治療,30min/次,每天兩次(上午、下午各一次)。兩組期間均不予其他止痛消腫藥物治療,共予以治療2周。兩組患者在治療期間均不使用其他止痛消腫類型的藥物治療。評價方法采用視覺模擬評分法(Visual Analogue Scale/Score,簡稱VAS)及患者患肢腫脹程度測量以及患者白細胞、CRP變化情況來評價療效。 結(jié)果： 1.中藥封包組與云南白藥膏組對閉合型股骨干骨折術后疼痛的緩解均有明顯療效,治療前后差異有統(tǒng)計學意義(p0.01),中藥封包組總有效率為86.67%,云南白藥膏組總有效率為60.00%,兩組差異有統(tǒng)計學意義(p0.05),疼痛方面封包組優(yōu)于云南白藥膏組; 2.兩組在對閉合型股骨干骨折術后患者軟組織腫脹緩解方面,中藥封包組有效率為90.00%,云南白藥膏組有效率為63.33%,且兩組差異有統(tǒng)計學意義(p0.05)； 3.治療組在對改善閉合型股骨干骨折術后患者的白細胞、C反應蛋白方面,優(yōu)于對照組(p0.05)。 結(jié)論： 中藥封包與云南白藥膏均可以有效緩解閉合型股骨干骨折患者術后疼痛、軟組織腫脹,改善術后患者機體白細胞及C反應蛋白值,而總體療效方面,中藥封包組明顯優(yōu)于云南白藥膏組,且臨床應用中未發(fā)現(xiàn)毒副作用,值得進一步研究和臨床推廣。
[Abstract]:Objective:
To observe the therapeutic effect of traditional Chinese medicine package combined with conventional anti-inflammatory drugs on the recovery of femoral shaft fracture after operation. It is expected that a combination of traditional Chinese and Western medicine should be provided for the rehabilitation of femoral shaft fractures in clinical treatment.
Method:
A total of 60 patients with femoral shaft fractures were randomly selected from the first hospital of Changde, Hunan. The patients were randomly divided into two groups according to the time of hospitalization and the order of hospitalization, that is, 30 cases in the treatment group (external application of traditional Chinese Medicine), and 30 cases in the control group (the external application of the white ointment in Yunnan). For three days after the operation, the conventional anti inflammatory drugs were given for three days, and the traditional Chinese medicine was treated with traditional Chinese medicine on the second day after the operation. Two times a day (morning, one in the afternoon), the control group was operated on the same way with the treatment group, and the conventional anti inflammatory drugs were given for three days after the operation. Second days after the operation, the pain and swelling of the fracture end of the patient was treated with Yunnan white ointment, 30min/ times, two times a day (morning, one in the afternoon). The two groups were not treated with other analgesic and swelling drugs for 2 weeks. The two groups were treated with no other analgesic and swelling types during the treatment. The efficacy was evaluated by Visual Analogue Scale/Score (VAS) and the degree of swelling of the affected limbs and the changes of leukocytes and CRP in patients.
Result:
1. the traditional Chinese medicine package group and Yunnan white ointment group had obvious curative effect on the pain relief after the closure of the femoral shaft fracture. The difference was statistically significant before and after treatment (P0.01), the total effective rate of the traditional Chinese medicine package group was 86.67%, the total effective rate of the Yunnan white ointment group was 60%, the two groups had statistical significance (P0.05), and the pain envelope group was superior to Yunnan. White ointment group;
2. in the two groups, the swelling of the soft tissue in patients with closed femoral shaft fracture was 90%, the effective rate of Yunnan white ointment group was 63.33%, and the difference between the two groups was statistically significant (P0.05).
3. the treatment group was better than the control group in improving white blood cells and C reactive protein in patients with closed femoral shaft fracture (P0.05).
Conclusion:
Traditional Chinese medicine package and Yunnan white ointment can effectively relieve postoperative pain, swelling of the soft tissue, improve the value of leukocyte and C reaction protein in the patients with closed femoral shaft fracture, and the overall effect of the patients is better than that in the white ointment group in Yunnan.
