【摘要】：背景:骨關(guān)節(jié)感染因骨特有的解剖生理特征,其治療成功率仍不理想。成功治療的關(guān)鍵在于早期診斷,因此尋求一個敏感度與特異度均較高的生物學(xué)標(biāo)志物十分必要。目的:系統(tǒng)評價降鈣素原對成人骨關(guān)節(jié)感染的診斷價值。方法:計算機檢索1990年至2017年6月Pubmed、Embase、The Cochrane library及中國知網(wǎng)、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、萬方數(shù)據(jù)庫發(fā)表的文獻(xiàn)。由2位評價員按照預(yù)先設(shè)定的納入與排除標(biāo)準(zhǔn)獨立篩選文獻(xiàn)、提取資料,利用診斷試驗質(zhì)量評價工具2評價納入研究的偏倚風(fēng)險后,采用Stata 14.0軟件進(jìn)行Meta分析,計算合并敏感度、特異度、陽性似然比、陰性似然比、診斷比值比,繪制受試者工作特征曲線并計算曲線下面積。結(jié)果與結(jié)論:(1)共納入10篇文獻(xiàn),包含893例患者;(2)降鈣素原診斷成人骨關(guān)節(jié)炎的合并敏感度、特異度、陽性似然比、陰性似然比、診斷比值比分別為0.82(95%CI=0.72-0.89)、0.89(95%CI=0.82-0.93)、7.20(95%CI=4.6-11.3)、0.20(95%CI=0.13-0.32)、35(95%CI=18-71)和0.92(95%CI=0.90-0.94);(3)綜上,降鈣素原在診斷成人骨關(guān)節(jié)炎中具有較高的診斷效能。
[Abstract]:Background: the success rate of osteoarticular infection is still not satisfactory because of its unique anatomical and physiological characteristics. The key to successful treatment lies in early diagnosis, so it is necessary to seek a biomarker with high sensitivity and specificity. Objective: to evaluate the diagnostic value of procalcitonin in adult osteoarthrosis. Methods: from 1990 to June 2017, the published articles of Pubmeda Embase library, Chinese Biomedical Literature Database and Wanfang Database were searched by computer. According to the pre-set inclusion and exclusion criteria, two evaluators independently sifted the literature, extracted the data, evaluated the bias risk of the study by using diagnostic test quality evaluation tool 2, and carried out Meta-analysis by Stata 14.0 software. The combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic ratio were calculated. The operating characteristic curve was drawn and the area under the curve was calculated. Results and conclusion: (1) Ten articles including 893 patients were included; (2) the combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio of procalcitonin in the diagnosis of adult osteoarthritis were 0.82 (95CI0.72-0.89) 0.89 (95CI0.82-0.93) 7.20 (95CI4.6-11.3) 0.20 (95CI0.13-0.32) 35 (95CI18-71) and 0.92 (95CI18-71) and 0.90-0.94); (3, respectively. Procalcitonin has high diagnostic efficacy in the diagnosis of adult osteoarthritis.
