【摘要】：[目的]探討持續(xù)冷敷應用于踝部骨折的臨床有效性。[方法]選擇本院骨科2012年1月~2015年1月收治的踝部骨折住院患者66例,隨機分為持續(xù)冷敷組與常規(guī)處理組。持續(xù)冷敷組在常規(guī)處理的基礎(chǔ)上,局部持續(xù)冷敷3 d,3 d后給予彈力繃帶固定直至皮紋癥出現(xiàn);常規(guī)處理組入院后抬高患肢,靜脈滴注甘露醇,局部間斷冷敷3 d,每次20 min每天3次。入院后第1、3、7 d,對患者進行疼痛評分(VAS)、白細胞計數(shù)、血沉、C-反應蛋白檢測,記錄皮紋癥出現(xiàn)時間。入院后第1~7 d,測量患者踝部周徑。[結(jié)果]入院后第7 d持續(xù)冷敷組VAS評分明顯低于常規(guī)處理組[(5.60±0.54)vs(6.01±0.32),t=2.493,P0.05];持續(xù)冷敷組白細胞計數(shù)、血沉、C-反應蛋白分別為(0.62±0.10)×10~(10)/L、(16.60±4.00)mm/h、(8.17±0.81)mg/L,明顯低于常規(guī)處理組的(0.65±0.16)×10~(10)/L、(21.30±8.40)mm/h、(9.00±1.23)mg/L,(t=4.310,t=4.010,t=3.110,P0.05);持續(xù)冷敷組的踝部腫脹幅度為(1.25±0.22)cm明顯小于常規(guī)處理組的(1.80±0.17)cm,(t=4.050,P0.05);持續(xù)冷敷組皮紋癥出現(xiàn)時間為(6.2±1.5)d明顯早于常規(guī)處理組的(8.5±2.3)d,(t=4.230,P0.05)。[結(jié)論]持續(xù)冷敷能有效緩解踝部骨折腫脹、減輕疼痛及炎癥反應,縮短皮紋癥出現(xiàn)時間。
[Abstract]:[objective] to investigate the clinical efficacy of continuous cold compress in the treatment of ankle fracture. [methods] 66 cases of ankle fracture admitted from January 2012 to January 2015 were randomly divided into continuous cold compress group and routine treatment group. On the basis of routine treatment, continuous cold compress group was treated with elastic bandage 3 days later until dermatoglyphics appeared, and patients in routine treatment group were treated with mannitol intravenously, after admission, the limbs were raised and mannitol was injected intravenously. Local intermittent cold compress for 3 days, 3 times per day for 20 min. Pain score (VAS), leukocyte count, erythrocyte sedimentation rate (ESR) and C-reactive protein (ESR) were measured on the 7th day after admission, and the onset time of dermatoglyphics was recorded. The ankle circumference was measured 7 days after admission. [results] the VAS score of the continuous cold compress group was significantly lower than that of the routine treatment group on the 7th day after admission [(5.60 鹵0.54) vs (6.01 鹵0.32) P 0.05], while the white blood cell count in the continuous cold compress group was significantly lower than that in the conventional treatment group. 琛，
