【摘要】：目的:探討局部應(yīng)用氨甲環(huán)酸(TXA)聯(lián)合腎上腺素在減少全髖關(guān)節(jié)置換術(shù)(THA)后失血量的有效性和安全性。方法:將90例準(zhǔn)備行THA的患者隨機分為2組:TXA聯(lián)合腎上腺素組(聯(lián)合組,n=45)以及對照組(TXA組,n=45)。聯(lián)合組向關(guān)節(jié)腔內(nèi)注射TXA和腎上腺素,TXA組向關(guān)節(jié)腔內(nèi)注射TXA。比較兩組患者術(shù)中失血量、術(shù)后引流量、總失血量、隱性失血量、血紅蛋白(Hb)下降值以及紅細胞比容(Hct)下降值,同時比較分析兩組患者輸血率以及并發(fā)癥的發(fā)生率。結(jié)果:TXA聯(lián)合腎上腺素可以明顯減少THA患者術(shù)后引流量、總失血量以及隱形失血量、輸血率、Hb下降值以及Hct下降值,差異均有統(tǒng)計學(xué)意義(P0.05),對于術(shù)中出血量,兩組之間差異并無統(tǒng)計學(xué)意義(P0.05)。兩組患者術(shù)后并發(fā)癥的發(fā)生率比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:TXA聯(lián)合腎上腺素對減少單側(cè)THA術(shù)后出血的療效明確且不會增加患者術(shù)后并發(fā)癥的發(fā)生。
[Abstract]:Objective: to investigate the efficacy and safety of local application of TXA combined with epinephrine in reducing blood loss after total hip arthroplasty (tha). Methods: 90 patients with tha were randomly divided into 2 groups: TXA + epinephrine group (TXA + epinephrine group) and control group (TXA group). TXA was injected into the articular cavity in the combined group and TXA was injected into the articular cavity in the adrenoceptor TXA group. The blood loss, postoperative drainage, total blood loss, recessive blood loss, hemoglobin (HB) and erythrocyte volume (HCT) were compared between the two groups. The rate of blood transfusion and the incidence of complications in the two groups were compared and analyzed. Results the volume of blood loss, total blood loss and invisible blood loss, blood transfusion rate, HB and HCT decreased significantly in patients with tha after operation (P0.05). There was no significant difference between the two groups (P0.05). There was no significant difference in the incidence of postoperative complications between the two groups (P0.05). Conclusion the effect of TXA combined with epinephrine on reducing bleeding after unilateral tha is clear and does not increase the incidence of postoperative complications.
