珊瑚羥基磷灰石聯(lián)合濃縮生長(zhǎng)因子修復(fù)兔顱骨缺損

【摘要】:目的骨缺損修復(fù)是臨床一大難題,異質(zhì)骨材料與生長(zhǎng)因子復(fù)合以提高骨缺損修復(fù)能力近年來成為研究熱點(diǎn)。文中旨在探討異質(zhì)骨材料中珊瑚羥基磷灰石(CHA)聯(lián)合濃縮生長(zhǎng)因子(CGF)修復(fù)骨缺損的效果。方法選24只新西蘭兔,隨機(jī)數(shù)字表法分為CHA/CGF+CHA組、CHA/CGF+自體骨組和CHA+自體骨組,每組8只,在每只兔的兩側(cè)顱骨上各制備1個(gè)直徑10mm圓形缺損。術(shù)后6周和12周取材,通過Micro-CT及組織學(xué)觀察評(píng)價(jià)骨修復(fù)效果。結(jié)果 Micro-CT結(jié)果顯示,CHA/CGF+CHA組:術(shù)后6周骨缺損區(qū)CHA孔隙中有較多的新骨形成;術(shù)后12周CHA孔隙中有大量的新骨形成,骨密度明顯增加,CHA部分降解吸收。CHA/CGF+自體骨組:術(shù)后6周鄰近骨床的CHA孔隙內(nèi)有少量新生骨長(zhǎng)入;術(shù)后12周CHA孔隙內(nèi)的新骨量明顯增加。CHA+自體骨組:術(shù)后6周自體骨與骨床交界處有骨痂形成,邊界模糊;術(shù)后12周自體骨與周圍骨床完全融合,邊界不清。CHA/CGF+CHA組術(shù)后6周和12周新骨量多于CHA+自體骨組[(39.00±7.61)mm3vs(32.12±6.55)mm3,(49.75±2.36)mm3vs(39.45±7.02)mm3,P0.05],骨密度多于CHA+自體骨組[(308.30±29.82)mg/m L vs(256.85±151.25)mg/m L,(389.00±31.87)mg/m L vs(302.53±127.05)mg/m L,P0.05],在術(shù)后12周,CHA/CGF+CHA組新骨量和骨密度與CHA+自體骨組差異無統(tǒng)計(jì)學(xué)意義(P0.05)。組織學(xué)觀察結(jié)果:CHA+自體骨組術(shù)后6周植入的自體骨與骨床之間有新骨形成,并有少量結(jié)締組織長(zhǎng)入;術(shù)后12周,自體骨與周圍骨床完全融合,邊界不清。CHA/CGF+CHA組術(shù)后6周骨缺損區(qū)CHA孔隙內(nèi)有散在的新骨形成;術(shù)后12周,CHA孔隙內(nèi)有大量新骨形成并相互融合,部分CHA降解吸收。CHA/CGF+自體骨組術(shù)后6周近骨床的CHA孔隙中有少量編織骨長(zhǎng)入;術(shù)后12周,新生骨質(zhì)及類骨質(zhì)面積增大,但缺損中央?yún)^(qū)域的CHA孔隙內(nèi)僅有結(jié)締組織充填,無新骨形成,CHA部分降解吸收。結(jié)論 CHA與CGF聯(lián)合使用能有效地促進(jìn)骨愈合,CHA/CGF復(fù)合人工骨是理想的骨移植替代材料。
[Abstract]:Objective Bone defect repair is a difficult problem in clinic. Heterogeneous bone materials combined with growth factor to improve the ability of bone defect repair has become a hot topic in recent years. The purpose of this paper is to investigate the effect of coralline hydroxyapatite (cha) combined with concentrated growth factor (CGF) in repairing bone defects in heterogeneous bone materials. Methods Twenty-four New Zealand rabbits were randomly divided into cha / CGF autogenous bone group and cha autogenous bone group with 8 rabbits in each group. A diameter 10mm circular defect was prepared on each side of the skull of each rabbit. The bone repair effect was evaluated by Micro-CT and histological observation at 6 and 12 weeks after operation. Results Micro-CT results showed that in CHA / CGF cha group, there were more new bone formation in cha pores in the defect area 6 weeks after operation, and a large number of new bone formation in cha pores at 12 weeks after operation. Bone mineral density increased significantly in cha partial degradation and absorption. Cha / CGF autogenous bone group: a small amount of new bone appeared in cha pore near the bone bed 6 weeks after operation. The volume of new bone in cha pore increased significantly at 12 weeks after operation. In cha autogenous bone group, callus formed at the junction of autogenous bone and bone bed 6 weeks after operation, and the boundary was blurred, 12 weeks after operation, autogenous bone was completely fused with surrounding bone bed. 杈圭晫涓嶆竻.CHA/CGF CHA緇勬湳鍚,
