【摘要】：目的研究膝關(guān)節(jié)局限性色素性絨毛結(jié)節(jié)性滑膜炎的發(fā)病及形態(tài)學(xué)特點(diǎn),探討其治療方案。方法回顧分析筆者自2007-06—2014-12診治的膝關(guān)節(jié)局限性色素性絨毛結(jié)節(jié)性滑膜炎19例,所有患者采用膝關(guān)節(jié)鏡下病灶切除,2例出現(xiàn)種植轉(zhuǎn)移的患者結(jié)合術(shù)后放療。結(jié)果術(shù)后所有患者均獲隨訪11~63個(gè)月,平均29.8個(gè)月,所有患者術(shù)后臨床癥狀緩解,無復(fù)發(fā)。末次隨訪時(shí)VAS評(píng)分、膝關(guān)節(jié)活動(dòng)度與術(shù)前比較有所改善,差異有統(tǒng)計(jì)學(xué)意義(t=0.023,P=0.002;t=0.073,P=0.036)。結(jié)論膝關(guān)節(jié)局限性色素性絨毛結(jié)節(jié)性滑膜炎形態(tài)各異,遇特殊情況時(shí)可以出現(xiàn)種植轉(zhuǎn)移,膝關(guān)節(jié)鏡下病灶切除臨床效果確切,出現(xiàn)轉(zhuǎn)移時(shí),需結(jié)合局部放射治療。
[Abstract]:Objective to study the pathogenesis and morphological features of localized pigmented villonodular synovitis of knee joint and to explore its treatment. Methods A retrospective analysis of 19 cases of localized pigmented villonodular synovitis of knee joint treated from June to December 2007 was carried out. All patients were treated with arthroscopic resection of lesions and 2 patients with implantation metastasis combined with postoperative radiotherapy. Results all the patients were followed up for 11 ~ 63 months with an average of 29.8 months. At the last follow-up, the VAS score and the range of motion of knee joint were improved compared with those before operation, and the difference was statistically significant (t = 0.023, P = 0.002, t = 0.073, P = 0.036). Conclusion Local pigmented villonodular synovitis of knee joint is different in shape and can be implanted and metastasized under special circumstances. The clinical effect of focal resection under knee arthroscopy is definite. When metastasis occurs, local radiotherapy should be combined.
