【摘要】：目的探究丙泊酚與吲哚美辛聯(lián)合使用對離體人肺內(nèi)小動脈血管張力影響。方法選取因肺腫瘤行肺葉切除手術(shù)的病人腫瘤周圍正常肺組織作為實驗標(biāo)本,體式顯微鏡下分離人肺內(nèi)小動脈將其分兩組,丙泊酚組和丙泊酚聯(lián)合吲哚美辛組,組內(nèi)分為去內(nèi)皮組和內(nèi)皮完整組。使用微血管張力測定技術(shù),丙泊酚組給予U46619使血管產(chǎn)生持續(xù)性收縮,張力平穩(wěn)后,采用累計加藥法加入丙泊酚(10~300μmol/L);丙泊酚聯(lián)合吲哚美辛組用吲哚美辛(100μmol/L)預(yù)孵育30 min后,給予U46619使血管產(chǎn)生持續(xù)性收縮,張力平穩(wěn)后,采用累計加藥法加入丙泊酚(10~300μmol/L),分別觀察不同濃度丙泊酚對預(yù)收縮的人肺內(nèi)小動脈張力的影響。結(jié)果丙泊酚組丙泊酚對U46619預(yù)收縮人離體肺內(nèi)小動脈產(chǎn)生低濃度(10~100μmol/L)收縮高濃度(100~300μmol/L)舒張雙相作用,且去內(nèi)皮組在低濃度產(chǎn)生收縮幅度(Emax=31.19±5.10%)高于內(nèi)皮完整組(Emax=30.44±2.92%),P0.05。丙泊酚聯(lián)合吲哚美辛組吲哚美辛可阻斷丙泊酚引起的收縮反應(yīng),引起舒張反應(yīng)最大舒張率內(nèi)皮完整組(Emax=98.72±0.34%)高于去內(nèi)皮組(Emax=94.56±0.53%),P0.05。結(jié)論丙泊酚對經(jīng)U46619收縮的人肺內(nèi)小動脈呈低濃度收縮高濃度舒張雙相作用,與吲哚美辛聯(lián)合應(yīng)用可以阻斷其引起的肺血管收縮。收縮作用可能與丙泊酚增強(qiáng)肺動脈平滑肌環(huán)氧合酶活性,使環(huán)氧合酶源性收縮因子生成增多有關(guān)。舒張作用部分由內(nèi)皮細(xì)胞參與。
[Abstract]:Objective to investigate the effect of propofol combined with indomethacin on vascular tension of isolated human pulmonary arterioles. Methods the normal lung tissues of patients undergoing lobectomy were divided into two groups: propofol group and propofol combined with indomethacin group. The patients were divided into two groups: the group without endothelium and the group with intact endothelium. Using microvascular tension measurement technique, propofol group was given U46619 to induce sustained vasoconstriction. After steady tension, propofol 10300 渭 mol / L propofol was added to propofol plus indomethacin group after 30 min preincubation with indomethacin 100 渭 mol / L). U46619 induced sustained vasoconstriction. After steady tension, the effect of propofol at different concentrations on the tension of precontracted small pulmonary artery was observed by adding propofol 10300 渭 mol 路L ~ (-1) 路L ~ (-1). Results in propofol group, propofol produced 100 渭 mol / L (100 渭 mol / L) contraction of U46619 precontracted pulmonary arterioles in vitro, and the contraction amplitude of endothelium-free group was 31.19 鹵5.10 (Emaxmax 31.19 鹵5.10), which was higher than that of intact endothelium group (30.44 鹵2.922mol / L). Propofol combined with indomethacin could block the contraction response induced by propofol, and the maximal relaxation rate was 98.72 鹵0.34 in the intact endothelium group, which was higher than that in the endothelium-free group (94.56 鹵0.53 vs 94.56 鹵0.53). Conclusion propofol has a low concentration of contraction and high concentration of diastolic biphasic effect on human pulmonary arterioles contracted by U46619. Combined with indomethacin can block the pulmonary vasoconstriction induced by propofol. The effect of propofol on pulmonary artery smooth muscle cyclooxygenase activity may be related to the increase of cyclooxygenase-derived contraction factor production. The relaxation was partly mediated by endothelial cells.
