【摘要】：目的:目前人工膝關(guān)節(jié)單髁置換術(shù)的手術(shù)適應(yīng)證與禁忌證存在許多爭(zhēng)議,大致圍繞在膝關(guān)節(jié)的活動(dòng)度、內(nèi)外翻畸形角度、外側(cè)間室是否磨損、髕股關(guān)節(jié)是否正常、疼痛程度、年齡以及體重等。本文目的是為了研究不同的膝內(nèi)側(cè)間室寬度是否會(huì)對(duì)單髁置換術(shù)后近期療效產(chǎn)生影響,術(shù)前膝關(guān)節(jié)內(nèi)側(cè)間隙測(cè)量的寬度是否可以作為單髁置換術(shù)前適應(yīng)證的考量。方法:納入2014年12月至2016年3月我院關(guān)節(jié)外科行單髁置換術(shù)并得到隨訪的膝內(nèi)側(cè)間室骨關(guān)節(jié)炎患者68例,置換前對(duì)患者行站立位雙下肢全長(zhǎng)片,通過(guò)影像讀片系統(tǒng)測(cè)得膝關(guān)節(jié)內(nèi)側(cè)間隙寬度,以寬度2mm為界將患者分為A組(膝關(guān)節(jié)內(nèi)側(cè)間隙寬度?2mm),B組(膝關(guān)節(jié)內(nèi)側(cè)間隙寬度(29)2mm),收集兩組患者單髁置換術(shù)前與近期末次隨訪的膝關(guān)節(jié)OKS評(píng)分、KSS臨床評(píng)分以及HS S評(píng)分,做比較并進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果與結(jié)論:兩組患者術(shù)后均無(wú)并發(fā)癥,兩組患者末次隨訪的膝關(guān)節(jié)評(píng)分均較置換前明顯提高(P(27)0.05),兩組置換前膝關(guān)節(jié)評(píng)分有明顯差異(P(27)0.05),兩組末次隨訪的膝關(guān)節(jié)評(píng)分有明顯差異(P(27)0.05),說(shuō)明術(shù)前膝關(guān)節(jié)內(nèi)側(cè)間隙寬度是影響單髁置換術(shù)后近期療效的因素,單髁置換前膝關(guān)節(jié)內(nèi)側(cè)間隙寬度變窄且?2mm或者內(nèi)側(cè)間隙消失存在明顯的“骨對(duì)骨征”的患者更適合行單髁置換術(shù)。
[Abstract]:Objective: at present, the indications and contraindications of single condyle replacement of artificial knee joint are controversial, which mainly focus on the range of motion of knee joint, the angle of varus, the wear of lateral interventricular, whether the patellofemoral joint is normal or not, and the degree of pain. Age and weight. The purpose of this study was to investigate whether different medial compartment widths of the knee would have an impact on the short-term outcome after monondylar replacement, and whether the width of the medial space of the knee joint could be considered as a pre-operative indication for monondylar replacement. Methods: from December 2014 to March 2016, 68 patients with osteoarthritis of medial compartment of knee were treated with single condylar replacement in our hospital from December 2014 to March 2016. The width of the medial space of the knee joint was measured by the image reading system. Patients were divided into group A (width of medial space of knee joint) and group B (width of medial space of knee joint: 292mm) according to width of 2mm. The clinical score and HS-S score of patients before and after monodondylar replacement were collected. Make comparison and statistical analysis. Results and conclusion: there were no postoperative complications in both groups. The knee joint score of the last follow-up in the two groups was significantly higher than that before the replacement. There was a significant difference in the knee joint score between the two groups before the replacement. There was a significant difference between the two groups in the knee joint score at the last follow-up. This indicated the width of the medial space of the knee joint before operation. Degree is the factor that affects the short-term curative effect after monondylar replacement. Patients with narrow width of medial space of knee joint before monondylar replacement and 2 mm or disappearance of medial space have obvious bone-to-bone sign which is more suitable for monondylar replacement.
