【摘要】：目的和意義：本研究采用“痔上粘膜套扎法”聯(lián)合“痔核基底套扎法”的痔瘡套扎術(shù)與傳統(tǒng)的內(nèi)痔貫穿結(jié)扎術(shù)進行對比觀察,觀察痔瘡套扎術(shù)治療Ⅱ、Ⅲ期內(nèi)痔的臨床療效,探討其治療機制,為臨床推廣應(yīng)用提供理論依據(jù)。研究方法：選擇符合研究對象的60例內(nèi)痔患者隨機分為兩組,治療組(“痔上粘膜套扎法”聯(lián)合“痔核基底套扎法”的痔瘡套扎術(shù))和對照組(傳統(tǒng)的貫穿結(jié)扎術(shù))各30例,觀察兩組總體療效、手術(shù)當天、術(shù)后第1、3、7天兩組患者術(shù)后肛門墜脹和便血積分分布情況、痔核脫落時間及隨訪術(shù)后6個月療效情況。結(jié)果：1.兩組總有效率對比情況：治療組治愈29例,好轉(zhuǎn)1例,對照組治愈28例,好轉(zhuǎn)2例,總有效率均為100%,兩組總療效比較無顯著性差異(P0.05)；2.兩組患者術(shù)后肛門墜脹情況：手術(shù)當天、術(shù)后第1、3天有顯著差異(P0.05),術(shù)后第7天無顯著差異(P0.05),提示：術(shù)后早期肛門墜脹治療組優(yōu)于對照組；3.兩組患者術(shù)后便血對比情況：兩組手術(shù)當天均無大出血,治療組術(shù)后第1、3、7天便血積分情況與對照組經(jīng)秩和檢驗結(jié)果顯示治療組術(shù)后便血情況優(yōu)于對照組(P0.05)；4.兩者患者痔核脫落時間對比情況：治療組為(6.93±1.413)天,對照組為(11.5±1.503)天,兩組患者有顯著差異(P0.05),治療組明顯優(yōu)于對照組；5.術(shù)后6個月療效隨訪情況：治療組未見復(fù)發(fā)和其他后遺癥,對照組有1例內(nèi)痔核萎縮不全,痔核無脫出,無便血,經(jīng)秩和檢驗,P0.05,治療組6個月與對照組療效相當,復(fù)發(fā)率低,無明顯統(tǒng)計學(xué)意義P0.15)。兩組患者術(shù)后6個月隨訪均未出現(xiàn)肛門狹窄和肛門控便功能障礙情況,無統(tǒng)計學(xué)意義。結(jié)論：痔瘡套扎術(shù)與貫穿結(jié)扎術(shù)治療Ⅱ、Ⅲ期內(nèi)痔有相同療效,但痔瘡套扎術(shù)后并發(fā)癥較輕、手術(shù)時間和痔核脫落時間較短、復(fù)發(fā)率低、手術(shù)操作簡單容易撐握。
[Abstract]:Objective and significance: to observe the clinical effect of hemorrhoid ligation with hemorrhoid mucosal ligation combined with hemorrhoid nucleus basal ligation and traditional internal hemorrhoids through ligation. To explore the therapeutic mechanism and provide theoretical basis for clinical application. Methods: sixty patients with internal hemorrhoids were randomly divided into two groups: treatment group (30 cases) and control group (30 cases). To observe the overall curative effect of the two groups, the distribution of anus bloating and hematochezia score, the time of hemorrhoid nucleus falling out and the curative effect of 6 months follow up on the day of operation and the 1st day and 7th day after operation. The result is 1: 1. There were 29 cases cured and 1 case improved in the treatment group, 28 cases in the control group and 2 cases in the control group. The total effective rate of the two groups was 100. There was no significant difference in the total effective rate between the two groups. There was significant difference between the two groups on the day of operation, on the 1st day after operation, on the 3rd day after operation, and on the 7th day after operation, there was no significant difference between the two groups (P 0.05). It was suggested that the treatment group was better than the control group in the early stage of postoperative anal distension. There was no severe bleeding in both groups on the day of operation, and the scores of hematochezia on the 1st day after operation in the treatment group were better than those in the control group by rank sum test. The results showed that the hematochezia in the treatment group was better than that in the control group (P 0.05). The time of prolapse of hemorrhoids was 6.93 鹵1.413 days in the treatment group and 11.5 鹵1.503 days in the control group. There was a significant difference between the two groups (P 0.05), and the treatment group was better than the control group (P 0.05). 6 months follow-up: there was no recurrence and other sequelae in the treatment group. In the control group, there was no atrophy of the internal hemorrhoids, no prolapse of the hemorrhoid nucleus and no bleeding in the stool. The curative effect of the treatment group was similar to that of the control group at 6 months and the recurrence rate was low. There was no significant statistical significance (P0. 15). There was no anal stricture and anorectal defecation dysfunction in 6 months after operation in both groups, and there was no significant difference between the two groups. Conclusion: hemorrhoids ligation has the same curative effect as penetrating ligation for internal hemorrhoids in stage 鈪，
