【摘要】：目的:通過中、遠期隨訪觀察,評價微創(chuàng)經皮縫合手術治療急性閉合性跟腱斷裂的臨床療效與價值。方法:隨訪我院自2012年9月—2017年1月33例接受微創(chuàng)手術治療的急性閉合性跟腱斷裂患者,平均隨訪2.02年(3個月—5年),以門診隨訪結合電話隨訪,通過觀察切口情況、活動功能情況、有無再斷裂發(fā)生,并應用AOFAS評分標準對術后療效進行評價。結果:術后4例失訪,隨訪時間3個月~5年,平均2.02年。3所有病例無深靜脈血栓發(fā)生,無腓腸神經支配區(qū)感覺異常,無切口愈合不良、感染、皮緣壞死等切口并發(fā)癥發(fā)生;隨訪期內無跟腱再斷裂發(fā)生病例。術后中遠期足踝外形、功能恢復好,療效評定為:隨訪時間3個月至1年共11例,AOFAS評分平均92.63分,其中優(yōu)4例,良4例,優(yōu)良率72.7%;隨訪時間1年以上共22例,AOFAS評分平均96.33分,其中優(yōu)19例,良2例,優(yōu)良率95.5%。結論:微創(chuàng)經皮縫合手術是治療急性閉合性跟腱斷裂較好的方法,術后中遠期并發(fā)癥少、外形美觀、功能恢復良好。
[Abstract]:Objective: to evaluate the clinical effect and value of minimally invasive percutaneous suture for acute closed rupture of Achilles tendon. Methods: from September 2012 to January 2017, 33 patients with acute closed Achilles tendon rupture received minimally invasive surgery were followed up in our hospital from September 2012 to January 2017. The patients were followed up for an average of 2. 02 years (3 months to 5 years). The activity function and the occurrence of rerupture were evaluated by AOFAS score. Results: the follow-up time was 3 months to 5 years. No deep venous thrombosis, no sensory abnormality in the sural nerve innervation area, no wound healing, infection, skin edge necrosis and other incision complications occurred in 4 cases. No recurrent rupture of Achilles tendon occurred during the follow-up period. The average score of AOFAS was 92.63 points in 11 cases from 3 months to 1 year, of which 4 cases were excellent, 4 cases were good, the excellent and good rate was 72.7%, and the average AOFAS score was 96.33 points in 22 cases during the follow-up period of more than one year. Among them, 19 cases were excellent, 2 cases were good, the excellent and good rate was 95.5%. Conclusion: minimally invasive percutaneous suture is a better method for the treatment of acute closed rupture of Achilles tendon.
