【摘要】：背景:自體脂肪組織沒有排異性,由于其來(lái)源相對(duì)豐富,成本相對(duì)低廉,移植后外觀相對(duì)自然,目前已成為近乎理想的組織填充物。目的:文章就近年來(lái)不同添加成分對(duì)脂肪細(xì)胞活性影響的研究進(jìn)展做一綜述。方法:以"Autologousadipose tissue;rate of survival;stromal vascular fraction;platelet-rich plasma;growth factor;adipose derived stem cells;leptin"為英文檢索詞,以"脂肪細(xì)胞,成活率,自體脂肪移植,基質(zhì)血管成分,富血小板血漿,生長(zhǎng)因子,脂肪來(lái)源干細(xì)胞,瘦素"為中文檢索詞。應(yīng)用計(jì)算機(jī)檢索中國(guó)知網(wǎng)全文數(shù)據(jù)庫(kù)、萬(wàn)方全文數(shù)據(jù)庫(kù)、維普期刊全文數(shù)據(jù)庫(kù)、Pub Med數(shù)據(jù)庫(kù)、Elsevier數(shù)據(jù)庫(kù)、Embase數(shù)據(jù)庫(kù)有關(guān)脂肪細(xì)胞活性的文獻(xiàn),保留47篇文獻(xiàn)進(jìn)一步總結(jié)分析。結(jié)果與結(jié)論:自體脂肪組織移植還存在一些缺陷,研究發(fā)現(xiàn)單純顆粒脂肪移植后存活率低于60%,移植后脂肪組織吸收率較高,可能出現(xiàn)囊腫、鈣化等一系列并發(fā)癥,這些都制約著自體脂肪移植的廣泛臨床應(yīng)用。國(guó)內(nèi)外學(xué)者一直在試圖通過改進(jìn)吸脂方法、凍存條件,或采用不同添加成分如富血小板血漿、血管基質(zhì)成分等等不同方法,以提高脂肪移植成活率。其中血管基質(zhì)成分、富血小板血漿、脂肪來(lái)源干細(xì)胞、A型肉毒毒素、部分細(xì)胞因子已有臨床研究證實(shí)其可行及有效性,瘦素、部分生長(zhǎng)因子及藥物的研究仍需從動(dòng)物模型轉(zhuǎn)入臨床實(shí)驗(yàn)的研究。
[Abstract]:Background: autogenous adipose tissue has no heterogeneity, because of its relatively rich sources, relatively low cost and relatively natural appearance after transplantation, autologous adipose tissue has become an almost ideal tissue filler. Aim: to review the effects of different additives on adipocyte activity in recent years. Methods: the Chinese key words were "Autologousadipose tissue rate of survival stromal vascular fractionator platelet-rich growth factor adipose derived stem cellsleptin" and "adipocyte, survival rate, autologous fat transplantation, matrix vascular component, platelet-rich plasma, growth factor, adipose derived stem cell, leptin". The full text database of China Zhiwang, Wanfang full text database and the full text database of Weip periodical are searched by computer. The database of Elsevier / Elsevier / Embase database is related to adipocyte activity, and 47 articles are further summarized and analyzed. Results and conclusion: there are some defects in autologous adipose tissue transplantation. It was found that the survival rate of autogenous fat grafts was lower than that of 60 grafts, the absorption rate of adipose tissue was higher after transplantation, and a series of complications, such as cysts, calcification and so on, might occur. All these restrict the wide clinical application of autologous fat transplantation. Scholars at home and abroad have been trying to improve the liposuction method, freezing conditions, or using different additives such as platelet-rich plasma, vascular matrix components and other different methods to improve the survival rate of fat transplantation. Among them, vascular matrix, platelet-rich plasma, adipose-derived stem cell type A botulinum toxin, some cytokines have been clinically proven to be feasible and effective, leptin, Some studies of growth factors and drugs still need to be transferred from animal models to clinical trials.
