乙酰肝素酶mRNA在乳腺癌中的表達(dá)及其與乳腺癌病理特征的關(guān)系

【摘要】:背景 乙酰肝素酶(HPA)是一種葡萄糖醛酸脂酶,其能夠在特定部位裂解硫酸乙酰肝素蛋白聚糖(heparin sulfate proteoglycan, HPS G)中的硫酸乙酰肝素(HS)鏈,進(jìn)而導(dǎo)致細(xì)胞外基質(zhì)(extra cellular matrix, ECM)和基底膜(basement membrane,BM)發(fā)生降解,促進(jìn)腫瘤的浸潤(rùn)、轉(zhuǎn)移與微血管形成。乳腺癌是我國比較多發(fā)的惡性腫瘤之一,目前國內(nèi)有關(guān)于HPA mRNA是否在乳腺癌中表達(dá)的情況未見報(bào)道,也無文獻(xiàn)報(bào)道其是否與腫瘤的侵襲性生長(zhǎng)、淋巴結(jié)轉(zhuǎn)移和血管形成有關(guān)。本研究采用原位雜交方法和腫瘤微血管免疫組織化學(xué)技術(shù)檢測(cè)50例乳腺癌組織中的HPA mRNA的表達(dá)和腫瘤微血管密度microvessel density, MVD),對(duì) HPA mRNA在乳腺癌組織中的表達(dá)及其臨床意義進(jìn)行探討。目的探討乙酰肝素酶(HPA) mRNA及CD34在乳腺癌組織中的表達(dá),分析HPAmRNA與乳腺癌病理特征及MVD的關(guān)系。方法收集新鄉(xiāng)醫(yī)學(xué)院第一附屬醫(yī)院2013年2~9月手術(shù)切除的50例乳腺癌組織標(biāo)本及50例相應(yīng)癌旁正常乳腺組織(距離腫瘤邊緣5cm以上)標(biāo)本,患者均為女性,年齡23~76歲,中位年齡56歲,患者術(shù)前均未接受放射治療、化學(xué)治療及內(nèi)分泌治療;術(shù)后常規(guī)病理切片均證實(shí)為乳腺癌。按國際抗癌協(xié)會(huì)分類標(biāo)準(zhǔn):Ⅰ級(jí)14例,Ⅱ級(jí)26例,Ⅲ級(jí)10例;Ⅰ期10例,Ⅱ24例,Ⅲ期16例;N018例,N120例,N29例,N33例。每例標(biāo)本均分為2份,一份用液氮保存,待提取組織RNA;另一份應(yīng)用40 g·L-1多聚甲醛固定,生物自動(dòng)脫水機(jī)處理后石蠟包埋,行免疫組織化學(xué)染色。采用RT-PCR檢測(cè)HPA mRNA的表達(dá)情況及免疫組織化學(xué)方法檢測(cè)CD34的表達(dá)情況。應(yīng)用SPSS19.0軟件進(jìn)行統(tǒng)計(jì)分析,計(jì)量資料以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,兩兩比較采用t檢驗(yàn),計(jì)數(shù)資料比較采用χ2檢驗(yàn),檢驗(yàn)水準(zhǔn)為α=0.05。結(jié)果通過統(tǒng)計(jì)分析對(duì)比,乳腺癌組織和癌旁正常乳腺組織中HPA mRNA陽性表達(dá)率分別為70.0%(35/50)和24.0%(12/50),乳腺癌組織中HPA mRNA陽性表達(dá)率顯著高于癌旁正常乳腺組織(P0.01); HPA mRNA的表達(dá)與乳腺癌組織學(xué)分級(jí)、淋巴結(jié)轉(zhuǎn)移、TNM分期及腫瘤侵襲程度有相關(guān)性(P0.05,P0.01),與患者的年齡無關(guān)(P0.05);CD34陽性表達(dá)主要位于血管內(nèi)皮細(xì)胞的胞膜和胞質(zhì),呈棕黃色;35例HPA mRNA陽性患者為MVD為58.20±18.56,15例HPA mRNA陰性患者M(jìn)VD為41.30±9.35, HPA mRNA陽性患者M(jìn)VD顯著高于HPA mRNA陰性患者(P0.01)。結(jié)論HPA mRN A在乳腺癌組織中的表達(dá)顯著增高;HPA mRNA表達(dá)陽性的乳腺癌組織內(nèi)微血管密度高于其陰性表達(dá)者,其能夠促進(jìn)乳腺癌新生血管形成HPA mRNA高表達(dá)參與乳腺癌發(fā)展過程,與乳腺癌進(jìn)展、轉(zhuǎn)移密切相關(guān),為臨床上的腫瘤篩選及預(yù)后提供生物學(xué)標(biāo)記作為依據(jù)。
[Abstract]:Background Heparinase (HPA) is a glucuronidase that cleavage heparin sulfate proteoglycan (HPSG) chain of heparin sulfate in a specific site. In turn, extracellular matrix extra cellular matrix (ECM) and basement membrane BM2 were degraded, which promoted tumor invasion, metastasis and microangiogenesis. Breast cancer is one of the most frequent malignant tumors in China. At present, there are no reports on whether HPA mRNA is expressed in breast cancer, or whether it is related to invasive growth, lymph node metastasis and angiogenesis. In this study, the expression of HPA mRNA and tumor microvessel density (MVD) in 50 cases of breast cancer were detected by in situ hybridization and immunohistochemical technique. The expression of HPA mRNA in breast cancer and its clinical significance were studied. Objective to investigate the expression of heparanase HPA mRNA and CD34 in breast cancer, and to analyze the relationship between HPA mRNA and pathological features and MVD in breast cancer. Methods 50 breast cancer tissues and 50 adjacent normal breast tissues (above the margin of 5cm) were collected from the first affiliated Hospital of Xinxiang Medical College from February to September 2013. The patients were all female, aged 2376 years. The median age was 56 years old. The patients were not treated with radiotherapy, chemotherapy and endocrine therapy before operation.
