【摘要】：目的探討踝關(guān)節(jié)滑液血管活性腸肽水平(VIP)與踝創(chuàng)傷性關(guān)節(jié)炎(PTAOA)關(guān)節(jié)軟骨破壞,影像學(xué)以及癥狀嚴(yán)重程度的相關(guān)性。方法選取在我院進(jìn)行踝關(guān)節(jié)鏡清理術(shù)或關(guān)節(jié)置換術(shù)的患者74例以及正常體檢者69例,術(shù)前抽取患者的血清和踝關(guān)節(jié)滑液,影像學(xué)分級(jí)采用改良踝關(guān)節(jié)Kellgren-Law-rence(KL)影像學(xué)評(píng)分系統(tǒng),患者癥狀功能評(píng)定采用TeenyWiss和AOFAS足踝功能評(píng)分系統(tǒng),軟骨組織學(xué)退變得分采用改良Mankin得分。應(yīng)用酶聯(lián)免疫吸附試驗(yàn)(ELISA)測(cè)定患者關(guān)節(jié)滑液VIP水平,炎癥因子IL-6水平以及軟骨退變相關(guān)因子Ⅱ型膠原C端肽(CTX-Ⅱ)。結(jié)果血清VIP水平在患者與正常對(duì)照組間比較差異無(wú)顯著性,關(guān)節(jié)滑液VIP水平與K-L分級(jí),改良Mankin得分以及IL-6和CTX-Ⅱ呈顯著負(fù)相關(guān)性,此外關(guān)節(jié)滑液VIP水平與TeenyWiss和AOFAS得分呈正相關(guān)。結(jié)論踝關(guān)節(jié)滑液VIP的低表達(dá)反映了PTAOA嚴(yán)重程度,可作為PTAOA預(yù)后進(jìn)展的標(biāo)記物。
[Abstract]:Objective to investigate the correlation between the level of vasoactive intestinal peptide (VIPP) in synovial fluid of ankle and the destruction of articular cartilage, imaging and severity of symptoms in patients with traumatic arthritis of ankle (PTAOAA). Methods 74 cases of ankle arthroscopic debridement or arthroplasty and 69 cases of normal physical examination were selected. The serum and synovial fluid of the patients were collected before operation. Teeny Wiss and AOFAS ankle function scoring system and modified Mankin score were used to evaluate the symptom function of the patients.
