【摘要】：觀察分析微創(chuàng)經(jīng)皮鋼板固定術(shù)(MIPPO)與交鎖髓內(nèi)釘(IMN)治療脛骨遠(yuǎn)端關(guān)節(jié)外骨折的臨床療效。以2011年2月~2015年12月期間筆者所在醫(yī)院骨科收治的134例符合脛骨遠(yuǎn)端關(guān)節(jié)外骨折診斷標(biāo)準(zhǔn)的患者作為研究對(duì)象,隨機(jī)分為MIPPO組(n=67)和IMN組(n=67),MIPPO組給予MIPPO治療,IMN組給予IMN治療;觀察兩組患者術(shù)中治療情況、術(shù)后臨床療效和并發(fā)癥等。結(jié)果,MIPPO組患者手術(shù)時(shí)間、住院時(shí)間較IMN組均顯著縮短,住院費(fèi)用較高(P0.05)。兩組術(shù)中出血量、愈合時(shí)間無明顯差異(P0.05)。MIPPO組患者和IMN組患者治療有效率均能達(dá)到90%以上,治愈率高,能夠有效減輕患者痛苦;MIPPO組患者并發(fā)癥發(fā)生率低(9.0%),無愈合延期、畸形等并發(fā)癥,預(yù)后情況較佳,優(yōu)于IMN組(P0.05)。MIPPO療效突出,并發(fā)癥少,能有效縮短手術(shù)時(shí)間并降低對(duì)骨組織的損傷,IMN法治療費(fèi)用低,不易感染,均對(duì)臨床具有重要的意義。
[Abstract]:To observe and analyze the clinical effect of minimally invasive percutaneous plate fixation (MIPPO) and interlocking intramedullary nail (IMN) in the treatment of extraarticular fractures of distal tibia. From February 2011 to December 2015, 134 patients with extraarticular fractures of distal tibia were randomly divided into MIPPO group and IMN group. The intraoperative treatment, postoperative clinical efficacy and complications were observed in the two groups. Results the operation time and hospitalization time in MIPPO group were significantly shorter than those in IMN group, and the hospitalization cost was higher than that in IMN group. There was no significant difference in the amount of bleeding and healing time between the two groups. The effective rate of treatment was above 90% in both the P0.05-MIPPO group and the IMN group, and the cure rate was high. The rate of complications in the MIPPO group could effectively alleviate the pain of the patients with MIPPO, and the incidence of complications was lower than 9.0% and there was no delay in healing. The complications, such as deformity, had a better prognosis than IMN group (P 0.05N. MIPPO), with less complications, which could effectively shorten the operation time and reduce the cost of IMN therapy for bone tissue injury. All of them were of great significance to clinical practice.
