【摘要】：目的:介紹一種微創(chuàng)中下面部除皺術(shù)——可吸收整形提升帶(微拉美)聯(lián)合PPDO(可吸收鋸齒線)改善鼻唇溝過深、皮膚及面部深層軟組織松弛下垂的提拉固定術(shù),為新型可吸收生物除皺材料在微創(chuàng)除皺術(shù)中的臨床應(yīng)用提供理論基礎(chǔ)和實(shí)踐依據(jù)。方法:根據(jù)診斷標(biāo)準(zhǔn)、納入標(biāo)準(zhǔn)及排除標(biāo)準(zhǔn)選取要求行中下面部除皺術(shù)的患者共116例,其中A組(58例)行傳統(tǒng)切口高位SMAS(表淺肌肉筋膜系統(tǒng))除皺術(shù),B組(58例)行微拉美聯(lián)合PPDO的微創(chuàng)除皺術(shù)。將兩組受試者術(shù)后3個(gè)月、6個(gè)月的皺紋嚴(yán)重程度評(píng)分和中下面部整體美容效果改善分級(jí)數(shù)據(jù)進(jìn)行收集整理后,行統(tǒng)計(jì)學(xué)分析比較,得出結(jié)論。結(jié)果:隨訪3個(gè)月、6個(gè)月兩組輕中度衰老患者中下面部年輕化效果改善差別無統(tǒng)計(jì)學(xué)意義(P0.05),A組重度衰老患者中下面部年輕化效果改善要好于B組,差別有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:對(duì)于輕中度皺紋的患者,微拉美聯(lián)合PPDO的微創(chuàng)除皺手術(shù)效果與傳統(tǒng)除皺手術(shù)效果無差異,且具有手術(shù)時(shí)間短、切口小、并發(fā)癥少等優(yōu)點(diǎn),可吸收整形提升帶使面部深層軟組織解剖復(fù)位的同時(shí)還能增加局部軟組織容量,PPDO在吸收過程中,刺激皮膚膠原蛋白的生成,使皮膚真皮層增厚收緊,達(dá)到光亮緊致的效果。作為一種新型微創(chuàng)除皺術(shù),有一定的臨床推廣價(jià)值。但對(duì)于重度皺紋的患者傳統(tǒng)手術(shù)中下面部年輕化改善效果優(yōu)于微拉美聯(lián)合PPDO微創(chuàng)除皺術(shù)。
[Abstract]:Objective: to introduce a minimally invasive lower facial rhytidectomy-absorbable plastic lifting band (micro-Latin America) combined with PPDO (absorbable serrated line) to improve the depth of nasolabial sulcus, skin and deep soft tissue flabby and saggy fixation. It provides a theoretical and practical basis for the clinical application of new absorbable biodegradable materials in minimally invasive rhytidectomy. Methods: according to the diagnostic criteria, inclusion criteria and exclusion criteria, 116 patients with middle and lower facial rhytidectomy were selected. Group A (58 cases) were treated with traditional incision high position SMASA (superficial muscle fascia system) and group B (58 cases) with micro-Latin America combined with PPDO. The score of wrinkle severity and the classification data of overall cosmetic effect of middle and lower face were collected and sorted after 3 months and 6 months after operation in the two groups, and then statistical analysis and comparison were carried out, and the conclusion was reached. Results: following up for 3 months, there was no significant difference between the two groups in the improvement of the lower part rejuvenation effect in the patients with mild and moderate aging. There was no significant difference between the two groups. The improvement of the lower part in the patients with severe aging was better than that in the B group, and the difference was statistically significant (P 0.05). Conclusion: for patients with mild and moderate wrinkle, the effect of micro-Latin America combined with PPDO is not different from that of traditional wrinkle removal, and it has the advantages of short operation time, small incision, less complications and so on. Absorbable plastic lifting band can also increase the local soft tissue capacity and stimulate the formation of collagen in the process of absorption, which can make the skin dermis thickened and tighten, and achieve the effect of bright and tight. As a new type of minimally invasive rhytidectomy, it has certain clinical value. But for the patients with severe wrinkles, the effect of lower part rejuvenation was better than that of microLatin America combined with PPDO minimally invasive rhytidectomy.
