人體第6、7、8肋肋軟骨三維測(cè)量及骨化研究

【摘要】:[目的]分析不同年齡不同性別人群第6、7、8肋肋軟骨三維(長(zhǎng)、寬、厚)情況及骨化程度,為臨床隆鼻及耳廓再造等選用肋軟骨術(shù)前評(píng)估及術(shù)后遠(yuǎn)期預(yù)后估計(jì)提供參考。[方法]回顧分析昆明醫(yī)科大學(xué)第二附屬醫(yī)院放射科2014年5月到2016年3月320例行胸部CT檢查的6歲到45歲患者,5歲為1個(gè)年齡組,分成8組:6-10歲,11-15歲,16-20歲,21-25歲,26-30歲,31-35歲,36-40歲,41-45歲。每組40例患者,男性20例,女性20例。運(yùn)用多層螺旋CT (MSCT)檢查及飛利浦工作站提供的多種圖像后處理技術(shù):容積再現(xiàn)(volume rendering technique,VRT)、最大密度投影法(maximum intensity projection, MIP)、最小密度投影法(minimum intensity projection, Min IP)、表面陰影顯示法(shaded surface display, SSD)、多平面重組(multiple planar re-construction, MPR)、曲面重組(Curved planar reformat, CPR)等,對(duì)第6、7、8肋肋軟骨進(jìn)行CT值測(cè)量,得出三維數(shù)值及骨化體積、肋軟骨體積等相關(guān)數(shù)據(jù),分析肋軟骨骨化程度。采用SPSS17.0軟件進(jìn)行統(tǒng)計(jì)學(xué)分析,觀察指標(biāo)以均數(shù)±標(biāo)準(zhǔn)差(xs)表示。以P0.05為差異具有統(tǒng)計(jì)學(xué)意義。[結(jié)果]數(shù)據(jù)統(tǒng)計(jì)結(jié)果提示,第6肋軟骨右側(cè)長(zhǎng)度為(101.359 0±13.8962)mm,寬度為(12.119±3.3645)mm,厚度為(8.647±1.6501)mm。第6肋軟骨左側(cè)長(zhǎng)度為(106.200±17.1694)mm,寬度為(12.373±3.312)mm,厚度為(8.744±1.4668)mm。第7肋軟骨右側(cè)長(zhǎng)度為(140.469±18.9291)mm,寬度為(11.55±2.878)mm,厚度為(7.636±2.4719)mm。第7肋軟骨左側(cè)長(zhǎng)度為(143.309±20.3027)mm,寬度為(11.666±3.1590)mm,厚度為(7.677±1.9541)mm。第8肋軟骨右側(cè)長(zhǎng)度為(106.39±39.105)mm,寬度為(8.448±2.7122)mm,厚度為(6.553±1.6221)mm。第8肋軟骨左側(cè)長(zhǎng)度為(107.86±42.212)mm,寬度為(8.422±2.7895)mm,厚度為(5.272±1.9027)mm。[結(jié)論]1.人體第6、7、8肋肋軟骨的三維測(cè)量數(shù)據(jù)變化以20歲為界。20歲以前,肋軟骨的三維值隨著年齡增長(zhǎng)而增大,20歲以后基本不變。在所觀察的肋軟骨中,第7肋肋軟骨具有最大的厚度,長(zhǎng)度和寬度,三維數(shù)據(jù)穩(wěn)定,骨化程度適中,可作為整形外科行鼻整形或耳再造首選肋軟骨。2.人體第6、7、8肋肋軟骨骨化從15歲開(kāi)始,骨化率和骨化程度隨年齡增長(zhǎng)而增加。第6、7、8肋肋軟骨程度從上到下依次遞減。3.從肋軟骨的體積考慮,建議10歲以后再行肋軟骨切取的整形手術(shù)。從骨化角度來(lái)看,到45歲,依舊可行肋軟骨切取的整形手術(shù),因年齡增長(zhǎng)而增加的骨化不是行肋軟骨切取整形手術(shù)的絕對(duì)禁忌癥。
[Abstract]:[objective] to analyze the three dimensional (length, width, thickness) and ossification degree of costal cartilage in the 6th and 8th costal cartilage of different ages and different genders, so as to provide reference for clinical evaluation of costal cartilage before operation and long term prognosis estimation for auricle reconstruction. [methods] from May 2014 to March 2016, 320 patients aged from 6 to 45 years old who received chest CT examination in Department of Radiology, second affiliated Hospital of Kunming Medical University were analyzed retrospectively. They were divided into 8 groups: 6-10 years old, 11-15 years old, 16-20 years old, 16-25 years old, 26-30 years old, 31-35 years old and 36-40 years old and 41-45 years old. There were 40 patients in each group, 20 males and 20 females. Using multi-slice spiral CT scan and various image post-processing techniques provided by Philips workstation: volume rendering technique, maximum intensity projection, minimum intensity projection, Min projection, shaded surface display. SSD, Multiplanar recombination multiple planar re-construction, MPRA, curved Surface recombination, Curved planar reformat, CPR), etc. The CT value of the 6th and 8th costal cartilage was measured, and the three-dimensional value, ossification volume and costal cartilage volume were obtained, and the ossification degree of the costal cartilage was analyzed. The statistical analysis was carried out by SPSS17.0 software, and the observed index was expressed as mean 鹵standard deviation xs. P0.05 as the difference was statistically significant. [results] the data showed that the length of the right side of the sixth costal cartilage was 101.359 0 鹵13.8962mm, the width was 12.119 鹵3.3645mm, and the thickness was 8.647 鹵1.6501mm. The left length, width and thickness of the sixth costal cartilage were 106.200 鹵17.1694 mm, 12.373 鹵3.312 mm and 8.744 鹵1.4668 mm. respectively. The length of the right side of the seventh costal cartilage was 140.469 鹵18.9291mm. the width was 11.55 鹵2.878mm. the thickness was 7.636 鹵2.4719mm. The left length of the seventh costal cartilage was 143.309 鹵20.3027 mm, the width was 11.666 鹵3.1590 mm, and the thickness was 7.677 鹵1.9541 mm. The length, width and thickness of the eighth costal cartilage were 106.39 鹵39.105 mm, 8.448 鹵2.7122 mm and 6.553 鹵1.6221 mm respectively. The left length of the eighth costal cartilage was 107.86 鹵42.212mm, the width was 8.422 鹵2.7895mm, the thickness was 5.272 鹵1.9027mmm. [conclusion] 1. The change of 3D measurement data of costal cartilage in the 6th and 8th chondrocytes of human body. Before the age of 20 years, the three-dimensional value of the costal cartilage increases with the increase of age, and after 20 years of age, the value of the costal cartilage is almost unchanged. Among the costal cartilage observed, the seventh costal cartilage has the largest thickness, length and width, the three-dimensional data is stable, and the ossification degree is moderate. It can be used as the first choice of costal cartilage for orthopedic surgery or ear reconstruction. The ossification rate and the degree of ossification of costal cartilage began at the age of 15 years, and the rate of ossification and the degree of ossification increased with age. The degree of costal cartilage decreased from top to bottom. In view of the volume of costal cartilage, it is recommended that plastic surgery should be performed after 10 years of age. From the point of view of ossification, the plastic surgery of costal cartilage removal is still feasible at the age of 45, and the increase of ossification due to age is not an absolute contraindication for the operation of costal cartilage removal.
