【摘要】：目的探討MRI導(dǎo)引下真空輔助旋切乳腺病灶活檢技術(shù)的可行性。方法對45例患者共45個乳腺可疑病灶行MRI導(dǎo)引下真空輔助旋切活檢。觀察其技術(shù)成功率和術(shù)后并發(fā)癥情況,并進(jìn)行隨訪。結(jié)果在45個乳腺可疑病灶中,經(jīng)MRI導(dǎo)引下真空輔助旋切活檢43個(43/45,95.56%)病灶成功取樣并獲取病理學(xué)診斷,2個(2/45,4.44%)病灶未獲取病理學(xué)診斷。穿刺操作用時(45.87±8.41)min。5個(5/45,11.11%)活檢病理為惡性的病灶,均與術(shù)后病理結(jié)果相符;38個(38/45,84.44%)活檢病理為良性的病灶,6個月后MRI隨訪均未見惡性表現(xiàn)。根據(jù)活檢病理結(jié)果指導(dǎo)14例(14/45,31.11%)患者調(diào)整了治療方案,其中5例(5/45,11.11%)惡性腫瘤患者得以早期確診,9例(9/45,20.00%)疑似惡性的良性腫瘤患者避免了外科手術(shù)。45例患者活檢術(shù)后均無感染及血腫等并發(fā)癥發(fā)生。結(jié)論 MRI導(dǎo)引下真空輔助旋切乳腺病灶活檢安全、有效地完成乳腺可疑病灶取樣,對乳腺可疑病灶患者的臨床治療方案具有指導(dǎo)作用。
[Abstract]:Objective to investigate the feasibility of vacuum assisted biopsy of breast lesions guided by MRI. Methods 45 patients with suspected breast lesions underwent vacuum assisted biopsy guided by MRI. The technical success rate and postoperative complications were observed and followed up. Results among the 45 suspected breast lesions, 43 lesions were successfully sampled and pathological diagnosis was obtained by MRI guided vacuum assisted biopsy (43 / 43 / 45 ~ 95.56), and 2 / 42 / 45 ~ 4.44) lesions were not diagnosed by pathology. The biopsy time was 45.87 鹵8.41)min.5 / 45 / 11.11) the pathology of biopsy was malignant, which was consistent with the postoperative pathological results, and 38 cases (38 / 45 ~ 84.44) were benign lesions, and no malignant findings were found in the follow-up of MRI 6 months later. According to the pathological results of biopsy, 14 patients were guided to adjust the treatment plan. Among them, 5 cases (5 / 45 / 11.11) of malignant tumors were diagnosed early, 9 cases (9 / 45%) were suspected of malignant benign tumors, and none of the 45 cases had complications such as infection and hematoma after operation. Conclusion vacuum assisted biopsy of breast lesions guided by MRI is safe and effective in the sampling of suspected breast lesions, which can guide the clinical treatment of patients with suspected breast lesions.
